Hello, Dr.
Hsu.
I have been suffering from balanitis for nearly a year, and it tends to recur easily.
After applying medication (Nizoral cream containing ketoconazole), it improves, but then it relapses about a week later.
Will circumcision completely resolve this issue? Is this recurrent condition likely to lead to penile cancer? Each time it flares up, I initially feel a sharp pain in the affected area, followed by redness and swelling after a while.
However, I have experienced sharp pain in the past without any issues.
Why is this happening? I recall you previously suggested that patients with balanitis could apply petroleum jelly for maintenance.
If I continue to experience relapses after using it, should I keep applying petroleum jelly? I would appreciate your guidance, as I am struggling and feeling quite distressed.

Hello, Vaseline can be used long-term.
Recurrent infections may lead to skin changes, including malignancies.
Circumcision can facilitate cleanliness, but personal hygiene is still essential.
If there are any unclear points, feel free to ask again for further discussion and research.
Online responses should only be used as a reference; it is necessary to visit a healthcare facility for medical attention.

Recurrent balanitis, or inflammation of the glans penis, can be a frustrating and painful condition for many men. It often presents with symptoms such as redness, swelling, and discomfort, particularly after episodes of irritation or infection. In your case, it seems that despite using topical antifungal treatments like Nizoral (ketoconazole), you are experiencing recurrent flare-ups. This raises the question of whether circumcision could be a definitive solution to your problem.
Circumcision, the surgical removal of the foreskin, is often considered for men who suffer from recurrent balanitis, especially when conservative treatments fail. The rationale behind this is that circumcision removes the area that may harbor irritants or pathogens, thus reducing the likelihood of future infections. Studies have shown that circumcision can significantly decrease the incidence of balanitis and other related conditions, such as phimosis (tight foreskin) and paraphimosis (inability to retract the foreskin).
However, it is essential to note that circumcision is not a guaranteed cure for everyone. Some men may continue to experience balanitis even after the procedure, particularly if there are underlying factors such as diabetes, poor hygiene, or skin conditions like eczema or psoriasis. Therefore, before deciding on circumcision, it is crucial to evaluate all potential contributing factors to your recurrent balanitis.
Regarding your concern about the risk of penile cancer, chronic inflammation and irritation of the glans can increase the risk of certain conditions, including squamous cell carcinoma of the penis. However, the overall risk is relatively low, and circumcision has been shown to reduce this risk significantly. It is important to maintain good hygiene and seek medical advice if you notice any persistent or unusual changes in the skin of the penis.
As for the use of petroleum jelly (Vaseline) as a moisturizer, it can be beneficial in some cases to protect the skin and maintain moisture. However, if you are still experiencing recurrent infections, it may be worth discussing with your healthcare provider whether to continue this practice or explore other options.
In summary, circumcision may provide a long-term solution for recurrent balanitis, especially if conservative treatments have failed. However, it is essential to consider all underlying factors and consult with a urologist or dermatologist who specializes in male genital health. They can provide a comprehensive evaluation and help you make an informed decision about the best course of action for your specific situation. In the meantime, maintaining good hygiene and following your doctor's recommendations for topical treatments can help manage your symptoms.
