Menstruation is delayed?
I often experience menstrual cycles that exceed 28 days.
Since the second half of last year, my periods have been occurring approximately every 45 days.
The most recent one was in January.
Since that January period, I have not had my period again until now in May.
I am certain that I am not pregnant, and I have sought treatment from traditional Chinese medicine, but it doesn't seem to have had much effect.
I am unsure what the issue might be.
Which specialty should I consult for this? Thank you!

You should consult a gynecologist, as it is likely that the ovaries are not responding well to the gonadotropins secreted by the pituitary gland, or there may be an abnormal ratio of follicle-stimulating hormone (FSH) to luteinizing hormone (LH).
This is commonly seen in polycystic ovary syndrome (PCOS), where menstrual cycles are infrequent and may take longer to occur.
The primary focus of treatment should be lifestyle modifications: controlling weight, increasing physical activity, and paying attention to diet (avoiding excessive artificial sweeteners).
Further management should be tailored according to the specific symptoms presented.

Irregular menstrual cycles can be concerning, especially when they deviate significantly from what is considered normal. A typical menstrual cycle ranges from 21 to 35 days, with an average of 28 days. However, cycles that are consistently longer than 35 days or that vary significantly in length can indicate underlying health issues. In your case, experiencing cycles that are 45 days apart and then not having a period for several months warrants further investigation.
There are several potential causes for irregular menstrual cycles, including hormonal imbalances, stress, significant weight changes, excessive exercise, polycystic ovary syndrome (PCOS), thyroid disorders, and other medical conditions. Given that you have already ruled out pregnancy, it is essential to consider these factors.
1. Hormonal Imbalances: The menstrual cycle is regulated by a complex interplay of hormones. Any disruption in this balance can lead to irregular periods. Conditions such as PCOS, which is characterized by hormonal imbalance and often presents with irregular periods, may be a possibility.
2. Thyroid Issues: The thyroid gland plays a crucial role in regulating metabolism and can significantly affect menstrual cycles. Both hypothyroidism (underactive thyroid) and hyperthyroidism (overactive thyroid) can lead to irregularities in menstruation.
3. Stress and Lifestyle Factors: High levels of stress, significant changes in weight (either gain or loss), and excessive physical activity can all impact your menstrual cycle. If you have experienced any major life changes or stressors, this could be a contributing factor.
4. Other Medical Conditions: Conditions such as endometriosis or uterine fibroids can also lead to irregular periods. These conditions may require specific treatments and can affect fertility.
Given your situation, it is advisable to seek medical help. You should consider scheduling an appointment with a healthcare provider who specializes in women's health, such as a gynecologist. They can perform a thorough evaluation, which may include a physical examination, blood tests to check hormone levels, and possibly imaging studies to assess the reproductive organs.
In your initial visit, be prepared to discuss your menstrual history in detail, including the frequency, duration, and any associated symptoms (such as pain or changes in discharge). This information will help your doctor determine the best course of action.
If you have already tried alternative treatments, such as traditional Chinese medicine, and have not seen improvement, it is essential to communicate this to your healthcare provider. They may suggest lifestyle modifications, hormonal treatments, or other interventions based on the underlying cause of your irregular periods.
In summary, while irregular periods can be common, especially during certain life stages (like adolescence or perimenopause), your specific situation—particularly the long intervals between periods—warrants a medical evaluation. A gynecologist will be best suited to help you understand the underlying causes and develop an appropriate treatment plan. Don't hesitate to reach out for help; addressing menstrual irregularities early can lead to better health outcomes.
