Left lower abdominal pain?

Hello Dr.
Dong: I experience a dull pain in my lower left abdomen during activities, and I also feel soreness in my left lower back and left hip.
However, when I press on my lower left abdomen, it doesn't hurt.
This symptom has been present for about six months, but recently it feels more severe than before.
Do I need surgery for this? Thank you for your guidance!

Hello! Thank you for reaching out with your concerns regarding your left lower abdominal pain. Based on your description, it seems that you have been experiencing discomfort in your left lower abdomen, which has been accompanied by pain in your left waist and hip area. You mentioned that this has been ongoing for about six months, and it has recently intensified. Importantly, you noted that pressing on your left lower abdomen does not elicit pain, which is a significant detail.

Given the complexity of abdominal pain, especially when it persists for an extended period, it is crucial to consider a variety of potential causes. The left lower abdomen houses several vital organs, including parts of the intestines, the left ovary (in females), and the left kidney. Pain in this area can arise from gastrointestinal issues, musculoskeletal problems, or even referred pain from other regions.

1. Potential Causes:
- Gastrointestinal Issues: Conditions such as diverticulitis, irritable bowel syndrome (IBS), or even constipation can lead to discomfort in the left lower abdomen. If you have experienced changes in bowel habits or associated symptoms like bloating or changes in stool consistency, these could be contributing factors.

- Musculoskeletal Pain: Since you mentioned pain radiating to your waist and hip, it is possible that this discomfort could be related to muscle strain or issues with the lumbar spine. Activities that involve twisting or heavy lifting can exacerbate such pain.

- Gynecological Concerns: If you are female, conditions such as ovarian cysts or endometriosis could also manifest as left-sided abdominal pain. These conditions often require evaluation by a gynecologist.

- Kidney Issues: Although you do not report pain upon palpation, kidney stones or infections can sometimes present with referred pain in the lower abdomen.

2. When to Seek Surgical Advice:
- If your pain worsens or is accompanied by other concerning symptoms such as fever, significant changes in bowel or urinary habits, or if you experience any acute episodes of severe pain, it is essential to seek medical attention promptly.

- Given that your symptoms have intensified, it would be prudent to consult a healthcare provider who can perform a thorough evaluation. This may include imaging studies such as an ultrasound or CT scan, as well as laboratory tests to rule out any underlying conditions.

3. Next Steps:
- I recommend scheduling an appointment with a gastroenterologist or a general surgeon who specializes in abdominal pain. They will be able to assess your symptoms in detail and determine if further diagnostic testing is necessary.

- Keep a record of your symptoms, including when they occur, their intensity, and any associated activities or dietary changes. This information can be invaluable during your consultation.

In summary, while it is not possible to definitively determine whether surgery is necessary without a comprehensive evaluation, your ongoing symptoms warrant further investigation. Early diagnosis and treatment can significantly improve your quality of life and prevent potential complications. Please do not hesitate to seek medical advice, as timely intervention is key in managing abdominal pain effectively. Thank you for your inquiry, and I wish you the best in your journey toward relief and recovery!
