Pale complexion during exercise?
Hello doctor, my blood pressure is usually not high.
After exercising, my hands turn pale, my face becomes very red, and I feel dizzy.
Could this be due to myocardial ischemia? Is myocardial ischemia related to coronary artery narrowing, or could there be other possible causes? Thank you.

Dr. Lin Jixiong reply Cardiology
Hello: The symptoms that occur after exercise do not necessarily indicate a problem with coronary artery stenosis.
They may be related to phenomena associated with an increased heart rate following exercise.
Sincerely, Dr.
Lin Chi-Hsiung, Department of Cardiology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info
Experiencing paleness, flushed face, and dizziness after exercise can be concerning, and it’s important to understand the potential underlying causes. While your blood pressure is typically normal, the symptoms you describe may indicate several possibilities, including issues related to cardiovascular health, hydration status, or even anxiety.
Possible Causes of Symptoms After Exercise
1. Cardiovascular Response: During exercise, your heart rate increases to supply more oxygen to your muscles. If there is any underlying issue, such as coronary artery disease or other heart conditions, the heart may struggle to meet the increased demand for blood flow. This could lead to symptoms like dizziness or paleness due to inadequate blood supply to the brain or other vital organs.
2. Hypoxia: If your heart or lungs are not effectively delivering oxygen to your body during exertion, you might experience symptoms of hypoxia. This can occur due to various reasons, including respiratory issues or cardiac problems. If your heart is not pumping effectively, it could lead to feelings of lightheadedness or a pale appearance.
3. Dehydration: Exercise increases fluid loss through sweat. If you are not adequately hydrated before or during exercise, it can lead to decreased blood volume, resulting in paleness and dizziness. Dehydration can also cause your heart to work harder, which may exacerbate any underlying cardiovascular issues.
4. Vasodilation and Blood Flow Redistribution: During exercise, blood vessels in the muscles dilate to increase blood flow, which can lead to a temporary decrease in blood flow to other areas, including the skin. This can cause a flushed face while the extremities may appear pale due to reduced blood flow.
5. Anxiety and Stress: Psychological factors can also play a role in how your body responds to exercise. Anxiety can lead to hyperventilation, which may cause dizziness and a feeling of lightheadedness. It can also affect blood flow and cause paleness.
Recommendations for Management and Follow-Up
1. Consult a Cardiologist: Given your symptoms, it is advisable to consult a cardiologist for a thorough evaluation. They may recommend tests such as an echocardiogram, stress test, or advanced imaging to assess your heart's function and rule out any significant issues like coronary artery disease.
2. Monitor Hydration: Ensure you are well-hydrated before, during, and after exercise. Dehydration can significantly impact your performance and overall well-being.
3. Gradual Increase in Activity: If you have been inactive for a while, gradually increasing your exercise intensity can help your body adapt better. Start with low-intensity activities and slowly build up to more vigorous exercises.
4. Breathing Techniques: If anxiety is a contributing factor, practicing relaxation techniques or breathing exercises before and during exercise may help manage your symptoms.
5. Keep a Symptom Diary: Documenting when your symptoms occur, their severity, and any associated activities can provide valuable information for your healthcare provider.
6. Lifestyle Modifications: Incorporating a balanced diet, regular exercise, and stress management techniques can improve your overall cardiovascular health.
Conclusion
While your symptoms may not necessarily indicate a severe heart condition, they warrant further investigation to ensure your cardiovascular health is not compromised. It’s essential to listen to your body and seek medical advice when experiencing unusual symptoms, especially after physical activity. By taking proactive steps, you can better understand your health and improve your exercise experience.
