Vaginal dryness during sexual intercourse?

I am 32 years old, 160 cm tall, and weigh 43 kg, which gives me a slender physique.
Since my first sexual experience in college, I have noticed that although I feel excited when my boyfriend teases me, there is no vaginal lubrication.
I generally do not have a strong sexual desire, but I do feel the need when teased.
However, I always feel dry, and only become slightly moist at climax.
Is this normal? Can it be improved?

Ling Hui, 30~39 year old female. Ask Date: 2002/03/05

Dr. Wei Fumao reply Obstetrics and Gynecology

Individuals with low body weight may have insufficient estrogen levels.
They can consider supplementation or applying a lubricant to the vagina before sexual activity.

Understanding vaginal dryness is crucial for many women, especially those who may experience discomfort during sexual activity. Vaginal dryness can be a common issue, and it can stem from various causes, including hormonal changes, medications, and psychological factors.

Causes of Vaginal Dryness
1. Hormonal Changes: One of the most common causes of vaginal dryness is a decrease in estrogen levels. This can occur during different life stages, such as menopause, postpartum, or while breastfeeding. Estrogen is essential for maintaining the vaginal lining's moisture and elasticity.

2. Medications: Certain medications, particularly antihistamines, antidepressants, and hormonal contraceptives, can contribute to dryness. If you are taking any medications, it might be worth discussing with your healthcare provider to see if they could be affecting your vaginal lubrication.

3. Psychological Factors: Stress, anxiety, and depression can impact sexual arousal and lubrication. Even if you feel aroused physically, psychological barriers can inhibit the body's natural response, leading to dryness.

4. Insufficient Foreplay: Sometimes, inadequate foreplay can lead to insufficient natural lubrication. Engaging in more extended foreplay can help increase arousal and, consequently, lubrication.

5. Lifestyle Factors: Dehydration, smoking, and excessive alcohol consumption can also contribute to vaginal dryness. Maintaining a healthy lifestyle can improve overall sexual health.

Solutions for Vaginal Dryness
1. Use of Lubricants: Water-based or silicone-based lubricants can significantly enhance comfort during sexual activity. They can help reduce friction and make the experience more enjoyable. It's essential to choose a lubricant that is compatible with condoms if you are using them.

2. Moisturizers: Vaginal moisturizers can be used regularly to maintain moisture levels. These products are different from lubricants and can be applied a few times a week to help alleviate dryness.

3. Hormonal Treatments: If hormonal changes are the cause, discussing options with a healthcare provider is essential. Hormone replacement therapy (HRT) or localized estrogen treatments (like vaginal creams or rings) can be effective in restoring moisture.

4. Increase Foreplay: Engaging in more extended foreplay can help enhance arousal and lubrication. This can involve kissing, touching, and other forms of intimacy that do not necessarily lead to intercourse.

5. Address Psychological Factors: If anxiety or stress is contributing to your dryness, consider speaking with a therapist or counselor. They can help address any underlying issues that may be affecting your sexual health.

6. Healthy Lifestyle Choices: Staying hydrated, eating a balanced diet, and avoiding smoking and excessive alcohol can improve overall health and potentially alleviate dryness.

Conclusion
Experiencing vaginal dryness, especially when you feel aroused, can be frustrating and uncomfortable. However, it is essential to understand that this is a common issue and can often be addressed with simple lifestyle changes, the use of lubricants, or medical treatments. If dryness persists or worsens, it is advisable to consult a healthcare provider for a thorough evaluation and personalized treatment options. Open communication with your partner about your needs and comfort can also enhance your sexual experiences and intimacy.
