Laser lipolysis scars
Hello, Doctor.
In July of this year, I underwent laser lipolysis for facial slimming, which resulted in three burn injuries on both cheeks.
The photo link is http://www.wretch.cc/album/album.php?id=rex08420&book=3.
It has been almost three months, and the original hospital stated they would take responsibility until it heals, but so far they have only provided me with whitening injections.
Currently, I am applying Medgel on the wounds and using scar patches.
I would like to ask the doctor: 1.
Based on the photos, what degree of burns do the left and right cheeks appear to be? 2.
Is it possible for them to heal without scarring? 3.
How should I manage this now and in the future? 4.
Which is more effective for scar treatment: Medgel or Antiscar Cream? 5.
Do scars require massage? Thank you for your assistance.

Dr. Wu Yuxin reply Dermatology
It takes approximately 6 months for a wound to mature and form a scar.
Currently, using Medgel for wound care and scar patches is appropriate.
It is necessary to wait until after the 6-month period to determine the appropriate treatment for the scar, as sometimes no treatment is needed at all.
As for over-the-counter scar ointments like Dermatix and Medgel, they are quite similar and can continue to be used.

When dealing with burns from laser lipolysis, it is essential to understand the nature of the injury and the appropriate treatment options to promote healing and minimize scarring. Based on the information provided, here are detailed answers to your questions:
1. Burn Classification: The classification of burns typically ranges from first to third degree. First-degree burns affect only the outer layer of skin (epidermis), causing redness and minor pain. Second-degree burns extend into the second layer (dermis), resulting in blisters and more significant pain. Third-degree burns penetrate through all layers of skin, potentially affecting underlying tissues. Based on your description and the provided photos, it is likely that your injuries are classified as second-degree burns, given the presence of blisters and the depth of the injury.
2. Scar Recovery: The potential for scar-free recovery largely depends on the severity of the burn and how well it is treated. Second-degree burns can heal without scarring if managed properly. Factors influencing scar formation include the depth of the burn, the area affected, your skin type, and how well you adhere to aftercare instructions. With appropriate treatment and care, there is a possibility of minimal to no scarring.
3. Current and Future Treatment: For the current treatment, continue using the Medgel (美德凝膠) and scar patches as they can help in the healing process. It is crucial to keep the area clean and moisturized. Avoid exposing the burn to direct sunlight, as UV exposure can worsen scarring. If the area becomes painful, red, or shows signs of infection (such as pus or increased swelling), seek medical attention promptly. In the future, after the initial healing phase (approximately 6 months), you may consider consulting a dermatologist or plastic surgeon for advanced scar management options, such as laser therapy or silicone gel sheets.
4. Comparing Scar Treatments: Both Medgel and Antiscar Cream are designed to aid in scar healing, but their effectiveness can vary from person to person. Medgel is known for its hydrating properties and may help in softening the scar tissue, while Antiscar Cream often contains ingredients aimed at reducing pigmentation and promoting skin regeneration. It may be beneficial to use both products in conjunction, but always consult with your healthcare provider before combining treatments to ensure compatibility and effectiveness.
5. Scar Massage: Massaging the scar tissue can be beneficial once the skin has healed sufficiently (usually after 6-8 weeks). Gentle massage can help break down collagen fibers that form during the healing process, which may reduce the appearance of scars. Use a moisturizing lotion or oil during the massage to prevent irritation. However, avoid massaging if the area is still sensitive or painful, and consult with your healthcare provider for personalized advice.
In summary, managing laser lipolysis burns requires careful attention to wound care, monitoring for signs of infection, and possibly consulting with specialists for scar treatment. With proper care, the chances of achieving minimal scarring are favorable. Always follow up with your healthcare provider for ongoing assessment and treatment adjustments as needed.
