Regarding discomfort in the scrotum and testicles (continued)

Thank you, Dr.
Hsu, for your response! Today, I went to the hospital for an ultrasound examination, and it seems that varicocele is acting up again! However, I am puzzled.
If varicocele is caused by the dilation of blood vessels leading to discomfort, why did the doctor prescribe medications like nicametate and Ginkgo biloba, which are vasodilators? Wouldn't that exacerbate the symptoms? The doctor also performed a semen analysis.
Aside from assessing sperm motility and count, can this test also indicate any inflammatory conditions similar to a prostate fluid examination?
I found out that I have a small kidney stone, but the doctor mentioned that it is located within the tissue and can coexist without issues.
Does this mean I don't need to drink more water to help expel it, as it has become part of my body?
Is it advisable for patients with varicocele to avoid excessive sexual activity or masturbation in a short period, as well as the habit of delaying ejaculation to prolong intercourse, in order to prevent congestive prostatitis and varicocele congestion? I forgot to mention last time that, in addition to warm sitz baths for two weeks, I had sexual intercourse with my partner twice within an hour on the day before experiencing soreness and discomfort! I hope Dr.
Hsu can provide further guidance...
Thank you.

Dr. Xu Weikai reply Urology

Anonymous: Hello, varicocele is caused by impaired blood flow.
Some physicians may use circulatory medications.
A semen analysis can determine if there is a semen infection.
Although kidney stones may currently be asymptomatic, it is still important to drink plenty of water to prevent the formation of new stones.
The correlation between varicocele and sexual activity is not significant.
Here is what you wrote: (I saw it but didn't understand) "In addition to warm sitz baths for two weeks, I had sexual intercourse with my partner twice within an hour on the day before experiencing discomfort!" If there are any unclear points, feel free to ask again, and we can discuss and research further.

Varicocele, a condition characterized by the enlargement of veins within the scrotum, can lead to discomfort and various complications, including infertility. Understanding the treatment options and lifestyle considerations is crucial for managing this condition effectively.

Firstly, it's important to clarify the rationale behind the prescription of vasodilators like nicametate and Ginkgo biloba. While varicocele is indeed associated with venous dilation, these medications are often prescribed to improve blood circulation and reduce discomfort. The idea is that by enhancing blood flow, these medications can alleviate some of the symptoms associated with varicocele, such as pain or heaviness in the scrotum. However, it is essential to monitor how your body responds to these medications, as individual reactions can vary.

Regarding the semen analysis, this test is indeed useful not only for assessing sperm count and motility but also for identifying potential signs of inflammation. While it does not directly test for prostate inflammation, abnormalities in the semen can sometimes indicate underlying issues, including infections or inflammatory processes in the reproductive system. If there are concerns about prostate health, a specific prostate examination or additional tests may be warranted.

As for the kidney stone, the fact that it is described as "long in the meat" suggests that it is likely not obstructive and can coexist without causing significant issues. However, maintaining adequate hydration is generally recommended to support kidney health and prevent the formation of new stones, even if the existing stone is not causing immediate problems.

When it comes to sexual activity, individuals with varicocele should be cautious. Engaging in frequent sexual activity or masturbation can lead to increased blood flow to the area, potentially exacerbating discomfort. It is advisable to avoid excessive or vigorous sexual activity, especially in the days leading up to or following any episodes of pain. The practice of "holding back" ejaculation to prolong sexual activity may also contribute to congestion in the pelvic area, which could lead to complications such as prostatitis or worsening of varicocele symptoms.

In addition to medication and careful management of sexual activity, lifestyle modifications can play a significant role in managing varicocele discomfort. Here are some recommendations:
1. Wear Supportive Underwear: Supportive undergarments can help alleviate discomfort by providing support to the scrotum and reducing the strain on the veins.

2. Avoid Prolonged Standing or Sitting: If your job requires long periods of standing or sitting, take regular breaks to move around and improve circulation.

3. Maintain a Healthy Weight: Excess weight can increase pressure on the pelvic veins, potentially worsening varicocele symptoms.

4. Stay Hydrated: Adequate hydration is essential for overall health and can help prevent kidney stone formation.

5. Consider Dietary Changes: A diet rich in fruits, vegetables, and whole grains can support vascular health. Limiting salt and processed foods may also be beneficial.

6. Regular Exercise: Engaging in regular physical activity can improve circulation and overall health, which may help alleviate some symptoms.

7. Consult with a Specialist: If symptoms persist or worsen, it may be necessary to consult a urologist or a specialist in male reproductive health for further evaluation and potential surgical options.

In conclusion, managing varicocele discomfort involves a combination of medication, lifestyle adjustments, and careful monitoring of symptoms. Open communication with your healthcare provider is essential to tailor a treatment plan that works best for you. If you have further questions or concerns, do not hesitate to reach out to your physician for personalized advice.
