Sexual dysfunction issues
Hello.
I am experiencing sudden erectile difficulties.
It's hard to feel aroused even when watching videos.
Sometimes the base of the erection is firm, but the glans seems to be soft and not engorged.
I am unable to perform, and pressing on the glans doesn't cause pain.
Additionally, I feel a cramping sensation in my testicles.
What could be the issue?

Dr. Liu Jiaju reply Urology
The causes of erectile dysfunction can be categorized into psychological and organic factors.
Psychological causes are often sudden, with varying severity, and there are no issues with morning or nighttime erections.
In contrast, organic causes tend to progressively worsen, with poor morning and nighttime erectile function as well.
It is advisable to seek a detailed evaluation and treatment from a urologist.

More Info
Erectile dysfunction (ED) is a common condition that affects many men at various stages of life. It can be caused by a variety of factors, including physical, psychological, and lifestyle issues. Based on your description, it seems you are experiencing difficulties with achieving and maintaining an erection, which can be distressing and may impact your quality of life.
Causes of Erectile Dysfunction
1. Physical Causes:
- Vascular Issues: Conditions that affect blood flow, such as atherosclerosis (hardening of the arteries), hypertension, and diabetes, can lead to ED. If blood cannot flow adequately to the penis, achieving an erection becomes difficult.
- Hormonal Imbalances: Low testosterone levels can contribute to ED. Hormonal changes can occur due to aging, certain medical conditions, or medications.
- Neurological Disorders: Conditions such as multiple sclerosis, Parkinson's disease, or spinal cord injuries can affect nerve signals from the brain to the penis.
- Medications: Some medications, particularly those for high blood pressure, depression, and anxiety, can have side effects that include ED.
2. Psychological Causes:
- Stress and Anxiety: Performance anxiety or stress related to work, relationships, or other life situations can lead to temporary or chronic ED.
- Depression: Mental health issues can significantly impact libido and sexual performance.
- Relationship Problems: Issues with a partner can also contribute to sexual dysfunction.
3. Lifestyle Factors:
- Smoking and Alcohol Use: Both can impair blood flow and lead to ED.
- Obesity: Excess weight can lead to health issues that contribute to ED.
- Lack of Physical Activity: A sedentary lifestyle can negatively affect overall health and contribute to ED.
Solutions and Management
1. Consult a Healthcare Provider: It is essential to speak with a healthcare professional who can evaluate your symptoms, perform necessary tests, and determine the underlying cause of your ED. This may include blood tests to check hormone levels, blood sugar, and cholesterol levels.
2. Lifestyle Modifications:
- Exercise Regularly: Physical activity can improve blood flow and reduce stress, which may help alleviate symptoms of ED.
- Healthy Diet: A balanced diet rich in fruits, vegetables, whole grains, and lean proteins can improve overall health and potentially reduce ED symptoms.
- Quit Smoking and Limit Alcohol: Reducing or eliminating these substances can improve vascular health and overall well-being.
3. Psychological Support: If psychological factors are contributing to your ED, consider speaking with a therapist or counselor who specializes in sexual health. Cognitive-behavioral therapy (CBT) can be particularly effective.
4. Medications: There are several medications available that can help with ED, such as phosphodiesterase type 5 inhibitors (e.g., Viagra, Cialis). These medications work by increasing blood flow to the penis and are typically effective for many men.
5. Other Treatments: Depending on the underlying cause, other treatments may include hormone therapy, vacuum erection devices, or penile injections.
6. Follow-Up: Regular follow-up with your healthcare provider is essential to monitor your condition and adjust treatment as necessary.
Conclusion
Erectile dysfunction can be a complex issue with multiple contributing factors. It is crucial to approach this condition holistically, considering both physical and psychological aspects. Seeking professional help is the first step toward understanding and managing your symptoms effectively. Remember, you are not alone, and many men experience similar issues. With the right support and treatment, it is possible to regain confidence and improve sexual health.
