Is it floaters or a brain issue?

I have been experiencing discomfort in my eyes lately, with intermittent flashes of fine or thick bright lines (either single or two to three lines) that have a directional quality, but they do not consistently appear from top to bottom or at a fixed angle.
Sometimes they appear as straight lines, and other times they are curved but not jagged.
These flashes usually occur in bright areas, especially while moving, and occasionally when using the computer.
They flash by and then disappear, only to reappear after several minutes or some time later, primarily in my right eye.
Concerned about potential retinal issues, I consulted an ophthalmologist to rule out retinal detachment.
My intraocular pressure is somewhat elevated, and the doctor mentioned that my description (I wasn't very clear due to the crowd) does not resemble typical floaters.
After returning home, I tried covering the eye where the symptoms frequently occur, and while covered, I did not experience any flashes.
They also did not occur in dim light, and they have not caused any significant disruption in my daily life.
Should I seek a second opinion from another specialist for further confirmation?

Hello: Since you have already had a fundus examination, there should generally be no issues.
If you still have concerns, you may consider consulting another ophthalmologist for a second opinion.
Wishing you peace and happiness.

Your symptoms of experiencing transient flashes of light or lines in your vision, particularly when in bright environments or while moving, can indeed be concerning. Given that you have already consulted an ophthalmologist who ruled out retinal detachment, it is essential to consider other potential causes, particularly those related to neurological issues.

The phenomenon you describe may be related to a type of visual disturbance known as "photopsia," which refers to the perception of flashes of light. This can occur due to various reasons, including but not limited to migraine aura, vitreous detachment, or even neurological conditions. Since you mentioned that these flashes are more prominent in your right eye and that they disappear quickly, it may suggest a benign cause, especially if they do not interfere with your daily activities.

However, the fact that you have high intraocular pressure is noteworthy. Elevated eye pressure can lead to conditions such as glaucoma, which can cause visual disturbances. It is crucial to monitor this condition closely, as untreated glaucoma can lead to vision loss. Therefore, it is advisable to follow up with your eye doctor regarding your eye pressure and any necessary treatments.

Given that your symptoms do not occur in the dark or when you cover the affected eye, this could indicate that the flashes are not purely ocular in nature. The absence of symptoms in these situations may suggest that the disturbances could be related to how your brain processes visual information rather than a direct issue with the eye itself.

In addition to the eye-related concerns, it is essential to consider neurological evaluations, especially if you experience other symptoms such as headaches, dizziness, or changes in cognition. Conditions like migraine with aura can present with visual symptoms without necessarily causing a headache every time. If you have a history of migraines, this could be a relevant factor.

I recommend that you consult a neurologist for a comprehensive evaluation. They may suggest tests such as a brain MRI or EEG to rule out any neurological conditions that could be contributing to your symptoms. It is also beneficial to keep a diary of your symptoms, noting when they occur, their duration, and any associated factors, such as stress or fatigue, which can help your healthcare provider in making a more accurate diagnosis.

In summary, while your symptoms may not currently be causing significant disruption to your daily life, it is prudent to pursue further evaluation to ensure that any underlying issues are identified and managed appropriately. Regular follow-ups with both your ophthalmologist and a neurologist will provide a comprehensive approach to your health and peace of mind.
