Sexual issues?
I used to have normal erections even with frequent masturbation, but over the past month, I've noticed that while I can initially achieve an erection before sex, it becomes soft after undressing.
When I try to get it up again, there’s little response, and even attempting to put on a condom leads to it going soft before I can even get it on.
Sometimes, I can maintain an erection from the start of intercourse to the end, but often it goes soft after just a few thrusts, and occasionally I ejaculate very quickly.
I have consulted a doctor who mentioned that psychological factors might be involved, and I have taken medication that has slightly improved the situation.
I also asked the doctor about the possibility of penile venous leakage, but he said that if that were the case, I would have difficulty achieving an erection throughout intercourse.
Recently, even when I masturbate while watching adult films, my response varies; sometimes I get a strong erection, but other times it becomes soft shortly after.
Occasionally, when I feel close to ejaculation, I notice that my erection starts to fade, and then I ejaculate.
What could be the underlying issue, doctors?

Erectile dysfunction (ED) is a complex issue that can stem from a variety of physical and psychological factors. Understanding the causes and potential solutions is crucial for men experiencing this condition. Based on the symptoms you've described, it seems that your situation may involve both psychological and physiological components.
Causes of Erectile Dysfunction
1. Psychological Factors: Anxiety, stress, and depression are significant contributors to ED. The pressure to perform during sexual activity can create a cycle of anxiety that exacerbates the problem. If you have been experiencing stress from work or personal life, this could be impacting your sexual performance. Additionally, past experiences, such as performance anxiety or fear of intimacy, can also play a role.
2. Physical Factors: Various physical conditions can lead to erectile dysfunction. These include:
- Cardiovascular Issues: Poor blood flow due to narrowed arteries can prevent adequate blood from reaching the penis.
- Hormonal Imbalances: Low testosterone levels can affect libido and erectile function.
- Nerve Damage: Conditions like diabetes can lead to nerve damage, affecting the signals necessary for an erection.
- Substance Use: Smoking and excessive alcohol consumption can impair blood flow and nerve function, leading to ED.
3. Lifestyle Factors: Your mention of smoking and irregular sleep patterns suggests that lifestyle choices may also be contributing to your erectile dysfunction. Smoking can damage blood vessels, reducing blood flow to the penis, while poor sleep can affect hormone levels and overall health.
Solutions for Erectile Dysfunction
1. Consult a Healthcare Professional: Since you've already seen a doctor, it's essential to continue this dialogue. If psychological factors are suspected, a referral to a mental health professional may be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and improve sexual performance.
2. Medication: If your doctor has prescribed medication, it's important to follow their advice. Medications like phosphodiesterase type 5 inhibitors (e.g., Viagra, Cialis) can help improve blood flow to the penis and facilitate erections. However, these should be used under medical supervision.
3. Lifestyle Changes:
- Quit Smoking: This can significantly improve blood circulation and overall health.
- Limit Alcohol: Reducing alcohol intake can help improve sexual function.
- Exercise Regularly: Physical activity can enhance blood flow and improve mood, both of which are beneficial for erectile function.
- Healthy Diet: A balanced diet rich in fruits, vegetables, whole grains, and lean proteins can support cardiovascular health.
4. Manage Stress: Techniques such as mindfulness, meditation, or yoga can help reduce anxiety and improve your mental state, which may positively impact your sexual performance.
5. Communication with Partner: Open communication with your partner about your feelings and experiences can reduce anxiety and improve intimacy, which may help alleviate some of the psychological barriers to achieving and maintaining an erection.
6. Regular Check-ups: Since you mentioned concerns about potential physiological issues like venous leakage, regular check-ups with a urologist can help monitor your condition and adjust treatment as necessary.
Conclusion
Erectile dysfunction is a common issue that can be influenced by a myriad of factors. By addressing both the psychological and physical aspects of your health, you can work towards improving your erectile function. Remember, it's essential to maintain an open line of communication with your healthcare provider and partner as you navigate this challenge. With the right approach, many men find that their erectile function can improve significantly over time.
