Mild pain during sexual intercourse?

Hello Doctor, I would like to ask a question.
Sometimes when I engage in sexual intercourse with my boyfriend, I experience slight pain in my right abdomen, but this does not happen often.
We use condoms.
Could this be related to the position we are in? (Usually, I am on top).
I feel embarrassed to ask this question.
Could you please provide an answer?

1.
It should be unrelated to posture.
2.
Intense sexual activity can lead to uterine contractions, causing abdominal pain; however, if the abdominal pain is severe, it is advisable to consult a gynecologist for a thorough examination.

Experiencing occasional abdominal pain during sexual activity can be concerning, but it is not uncommon. There are several potential reasons for this discomfort, and understanding them can help you determine whether it's something to be worried about or if it’s a normal response to sexual activity.

Firstly, the positioning during intercourse can indeed play a role in causing discomfort. In your case, the "woman on top" position may lead to deeper penetration, which can sometimes cause pressure on the abdominal organs, particularly if the thrusting is vigorous. This can result in a sensation of pain or discomfort in the lower abdomen, especially if there is any underlying tension in the pelvic muscles or if the angle of penetration is not optimal for your anatomy.

Another possibility is that the pain could be related to the pelvic floor muscles. These muscles can become tense or spasm during sexual activity, particularly if you are feeling anxious or if there is a lack of relaxation. This tension can lead to discomfort in the abdomen. Additionally, if you are not fully aroused or if there is insufficient lubrication, this can also contribute to discomfort during intercourse.

In some cases, the pain could be related to underlying medical conditions. For instance, conditions such as endometriosis, pelvic inflammatory disease, or ovarian cysts can cause pain during sexual activity. If the pain is persistent, severe, or accompanied by other symptoms such as unusual bleeding, changes in menstrual cycles, or gastrointestinal symptoms, it would be advisable to consult a healthcare professional for further evaluation.

Moreover, if you have a history of any gynecological issues, such as fibroids or previous surgeries, these could also contribute to discomfort during sex. It’s important to communicate openly with your partner about any discomfort you experience, as this can help both of you adjust your activities to ensure a more comfortable experience.

If the pain is infrequent and mild, it may not be a cause for concern. However, if you find that the pain persists or worsens, it would be wise to seek medical advice. A healthcare provider, particularly a gynecologist, can perform a thorough examination and may recommend imaging studies or other diagnostic tests to rule out any underlying conditions.

In summary, occasional abdominal pain during sexual activity can arise from various factors, including positioning, muscle tension, or underlying medical conditions. If the discomfort is mild and infrequent, it may not be a significant issue. However, if you have concerns or if the pain persists, seeking medical advice is the best course of action to ensure your health and well-being. Remember, open communication with your partner and healthcare provider is key to addressing any concerns you may have regarding your sexual health.
