Do I need to get orthodontic treatment?

Hello Doctor: Last week, I had a fracture on the first molar on the upper left side due to eating (the fracture is on the inner side and accounts for about one-third of the tooth).
I visited a dental clinic where the dentist removed the fractured part (as it had not completely fallen out) and smoothed the tooth to avoid occlusion.
The dentist then explained that I would need orthodontic surgery to pull the tooth upwards from the base, which would take about three months, after which a single dental crown would be placed.
My question is whether it is possible to skip the orthodontic surgery and directly place a single dental crown instead.
I would appreciate your response to this question.
Thank you!

The purpose of corrective surgery is to achieve a banding effect, allowing braces to be worn for an extended period.
However, as mentioned in the text, one could consider directly using resin to fill the gaps, providing a one-time solution to the problem.

When it comes to orthodontic treatment following tooth extraction, particularly in your case where a significant portion of a tooth has been removed, there are several factors to consider. First, let's clarify the purpose of orthodontic treatment in this context. Orthodontics is primarily aimed at correcting misaligned teeth and improving bite function. When a tooth is extracted, the surrounding teeth may shift over time, leading to misalignment and potential bite issues.
In your situation, the dentist has recommended orthodontic treatment to reposition the adjacent teeth before placing a dental crown (or prosthetic tooth) on the extracted site. This approach is often beneficial because it ensures that the new crown will fit properly within the dental arch and align correctly with the opposing teeth. If the adjacent teeth are allowed to shift without correction, it may lead to complications such as improper bite, increased wear on the teeth, and even jaw pain.

However, you are asking whether it is possible to skip the orthodontic treatment and directly place a dental crown. The answer is that it may be possible, but it is not generally advisable. If the adjacent teeth are not properly aligned, the crown may not fit well, leading to discomfort and potential complications down the line. Additionally, if the teeth shift significantly, you might find yourself needing further orthodontic treatment later on, which could be more extensive and costly.

It's also important to consider the long-term implications of not addressing the alignment issue. Misalignment can lead to uneven wear on your teeth, increased risk of cavities, and even periodontal disease due to difficulty in maintaining proper oral hygiene. Furthermore, if the teeth shift too much, it could create spaces that are difficult to manage with a crown alone.

In some cases, if the misalignment is minimal and the dentist believes that the crown can be placed without significant issues, they might proceed without orthodontic treatment. However, this decision should be made with careful consideration and ideally, a second opinion from an orthodontist or a prosthodontist (a dentist specializing in crowns and bridges) could provide additional insights.

In summary, while it may be tempting to skip the orthodontic treatment to expedite the process of getting a dental crown, it is generally in your best interest to follow the dentist's recommendation. Proper alignment will not only ensure that the crown fits well but will also contribute to your overall oral health in the long run. If you have concerns about the treatment plan or the necessity of orthodontics, don’t hesitate to discuss these with your dentist or seek a second opinion. Your comfort and understanding of the treatment process are crucial for achieving the best outcome for your dental health.
