Phimosis issue
Hello Doctor, I previously had a problem with phimosis and excessive foreskin, but during puberty, I attempted to retract the foreskin and everything was fine at that time.
Sexual intercourse was normal, but in the past six months, I suddenly experienced pain when trying to retract the foreskin after an erection.
I didn't think much of it and continued to have sexual relations with my girlfriend, but the pain has become increasingly severe.
When I slowly retract the foreskin after an erection, I notice many tears on the foreskin, and there is even some bleeding.
It especially hurts when it comes into contact with water.
What should I do to treat this condition? Are there any medications that can help? Thank you.
Xu, 20~29 year old female. Ask Date: 2013/02/11
Dr. Xu Weikai reply Urology
Hello, it seems you may have a case of balanitis, which is causing localized swelling, pain, and tearing.
I recommend seeking medical attention as soon as possible.
Local medications are not advised for self-use.
If you really prefer not to see a doctor, you can use petroleum jelly as a maintenance treatment.
If you have any further questions or need clarification, feel free to ask, and we can discuss it further.
Please remember that online responses should only be used as a reference; it is essential to visit a healthcare facility if necessary.

More Info
Phimosis is a condition where the foreskin cannot be easily retracted over the glans (the head of the penis). It can be a source of discomfort and pain, especially during erections or sexual activity, as you've described. Your symptoms, including pain, tearing, and bleeding, suggest that you may be experiencing a more severe form of phimosis, possibly complicated by inflammation or infection.
Symptoms of Phimosis
The symptoms of phimosis can include:
- Difficulty retracting the foreskin, especially during an erection.
- Pain or discomfort when attempting to retract the foreskin.
- Tearing of the foreskin, which can lead to bleeding.
- Swelling or inflammation of the foreskin.
- Pain during urination or sexual activity.
Treatment Options
1. Topical Steroid Creams: One of the first-line treatments for phimosis is the application of topical steroid creams, such as betamethasone or hydrocortisone. These creams can help to reduce inflammation and make the skin more pliable, which may allow for easier retraction of the foreskin over time. It is typically recommended to apply the cream twice daily for a few weeks.
2. Gentle Stretching Exercises: Along with topical treatments, gentle stretching of the foreskin can be beneficial. This should be done carefully to avoid further tearing or injury. It’s important to perform these stretches when the skin is warm, such as after a shower, to minimize discomfort.
3. Circumcision: If conservative treatments fail or if the phimosis is severe, circumcision may be recommended. This surgical procedure involves the removal of the foreskin and is considered a definitive treatment for phimosis. It is usually performed under local anesthesia and has a high success rate.
4. Preputioplasty: This is a less invasive surgical option that involves widening the opening of the foreskin without removing it. This can be an option for those who wish to retain their foreskin but still need relief from the symptoms of phimosis.
5. Antibiotics: If there is an infection present (e.g., balanitis), antibiotics may be prescribed to treat the infection before addressing the phimosis itself.
Care and Management
- Hygiene: Maintaining proper hygiene is crucial. Gently wash the area with mild soap and water, and avoid using harsh chemicals or irritants.
- Avoid Irritants: Be cautious with products that may irritate the genital area, such as scented soaps or lotions.
- Follow-Up: Regular follow-up with a healthcare provider is important to monitor the condition and adjust treatment as necessary.
When to Seek Medical Attention
Given your symptoms of tearing and bleeding, it is advisable to seek medical attention promptly. A healthcare provider can assess the severity of your condition, rule out any infections, and recommend the most appropriate treatment plan tailored to your needs.
In summary, while there are effective treatments available for phimosis, it is essential to consult with a healthcare professional for a proper diagnosis and treatment plan. Avoiding further irritation and injury to the area is crucial, and timely intervention can help prevent complications.
