Experiencing pain while using the restroom during menstruation?

Hello, doctor.
I would like to ask about a symptom I've been experiencing for the past six months.
During my menstrual period, I feel pain in my anus when I have a bowel movement or pass gas, but it's not the same as menstrual cramps.
I do experience menstrual cramps, but they usually only occur on the first day of my period.
The pain in my anus only happens during bowel movements or when passing gas, and my menstrual cycle is normal, lasting about seven days.
During my period, I do not experience any pain while having a bowel movement.
What could this symptom indicate? Is there a possibility that it could lead to infertility in the future?

Niming, 10~19 year old female. Ask Date: 2013/02/16

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, based on your description of symptoms, it seems similar to the phenomenon of "urgency and heaviness" during menstruation.
I recommend that you consult a specialist in obstetrics and gynecology to evaluate whether there is endometriosis or other conditions.
Thank you! Wishing you peace and health, Dr.
Zhang.

Understanding pain during bowel movements, especially in relation to menstruation, can be a complex issue that involves various physiological factors. Based on your description, it seems that you experience pain in the anal area specifically during bowel movements or when passing gas, which is distinct from the menstrual cramps you experience at the onset of your period.
This type of pain can be attributed to several potential causes. One common reason for anal pain during bowel movements is the presence of hemorrhoids, which are swollen veins in the lower rectum and anus that can become inflamed and painful, particularly during straining. Another possibility is anal fissures, which are small tears in the lining of the anus that can occur due to hard stools or straining during bowel movements. Both conditions can be exacerbated by hormonal changes during menstruation, which can affect bowel habits and lead to constipation or diarrhea.

Additionally, the menstrual cycle can influence the gastrointestinal tract due to hormonal fluctuations. Prostaglandins, which are released during menstruation, can cause increased bowel motility and cramping, potentially leading to discomfort during bowel movements. This could explain why you experience pain specifically during this time.

Regarding your concern about infertility, it is important to note that pain during bowel movements related to menstruation is typically not directly linked to infertility. However, if the pain is associated with underlying conditions such as endometriosis, which can cause pelvic pain and may affect fertility, it would be advisable to consult a healthcare provider for further evaluation. Endometriosis occurs when tissue similar to the lining inside the uterus grows outside of it, and it can lead to various symptoms, including painful bowel movements, especially during menstruation.

To manage your symptoms, consider the following recommendations:
1. Dietary Adjustments: Ensure you are consuming a high-fiber diet to promote regular bowel movements and prevent constipation. Staying hydrated is also crucial.

2. Over-the-Counter Medications: If you suspect hemorrhoids or anal fissures, topical treatments such as hydrocortisone creams or witch hazel pads may provide relief. For pain management, non-steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help alleviate discomfort.

3. Warm Baths: Soaking in a warm bath can help relax the anal muscles and reduce pain during bowel movements.

4. Consult a Healthcare Provider: If the pain persists or worsens, it is essential to seek medical advice. A healthcare provider can perform a thorough examination to rule out any serious conditions and provide appropriate treatment options.

5. Pelvic Floor Therapy: If you experience significant pain, pelvic floor therapy may be beneficial. A specialized physical therapist can help address any muscle tension or dysfunction contributing to your symptoms.

In conclusion, while the pain you are experiencing during bowel movements related to your menstrual cycle is likely not a direct cause of infertility, it is essential to monitor your symptoms and consult a healthcare professional for a comprehensive evaluation. Early intervention can help manage your symptoms effectively and ensure your overall reproductive health remains intact.
