Post-Surgery Swelling and Pain: What to Do After Varicocele Surgery? - Urology

I would like to ask the doctors, what should be done about swelling and pain near the incision site after surgery?

Hello, doctor: At the end of November last year, I underwent bilateral varicocele surgery at Tzu Chi Hospital due to pain from testicular varicocele.
After the surgery, I experienced swelling and pain in the area near the incision (where pubic hair grows).
The doctor mentioned that this was due to the surgical incision and advised me to apply ice to reduce the swelling.
However, it has been nearly three months since the surgery, and the swelling has not subsided, and the pain persists.
Ice application does not seem to have a significant effect.
I returned to see the doctor, who stated that typically, there should not be swelling or pain near the incision after varicocele surgery, and again advised ice application, mentioning that there are no medications to treat this.
Is my swelling normal? Besides ice application, are there any other methods to reduce swelling and alleviate pain? If I need to visit a larger hospital, which department should I consult? I would appreciate the doctors' guidance, as I am feeling quite distressed.
Thank you!

Post-surgery swelling and pain, especially after a varicocele surgery, can be concerning and frustrating for patients. It is important to understand that while some degree of swelling and discomfort is expected after such procedures, persistent symptoms that last for months warrant further investigation.

Understanding Post-Surgery Symptoms
After varicocele surgery, it is common for patients to experience some swelling and pain around the surgical site. This is due to the body’s natural healing process, which involves inflammation and fluid accumulation. However, if the swelling and pain persist for three months, as in your case, it may indicate complications or issues that need to be addressed.

Possible Causes of Persistent Swelling and Pain
1. Hematoma Formation: A hematoma is a localized collection of blood outside of blood vessels, which can occur after surgery. This can lead to swelling and pain and may require drainage if it does not resolve on its own.

2. Infection: Although you may not have signs of infection such as fever or discharge, it is still possible to have a low-grade infection that could cause prolonged swelling and discomfort.

3. Nerve Irritation: Surgical procedures can sometimes irritate nearby nerves, leading to chronic pain or discomfort in the area.

4. Lymphatic Obstruction: Surgery can disrupt lymphatic drainage, leading to lymphedema, which is swelling due to lymph fluid accumulation.

5. Scar Tissue Formation: Excessive scar tissue can form after surgery, which may lead to pain and discomfort.

Recommended Actions
1. Follow-Up with Your Surgeon: Since your symptoms have persisted, it is crucial to return to your surgeon or seek a second opinion. They may recommend imaging studies, such as an ultrasound, to assess for hematomas or other complications.

2. Pain Management: If ice packs are not providing relief, discuss alternative pain management options with your doctor. This may include over-the-counter pain relievers like ibuprofen or acetaminophen, or prescription medications if necessary.

3. Physical Therapy: Sometimes, physical therapy can help alleviate pain and improve function. A physical therapist can provide exercises to strengthen the area and improve circulation.

4. Consult a Specialist: If your surgeon is unable to provide answers, consider consulting a urologist or a pain management specialist. They can offer insights into your symptoms and potential treatments.

5. Lifestyle Modifications: Gentle activities, avoiding heavy lifting, and wearing supportive underwear may help reduce discomfort during the healing process.

Conclusion
It is understandable to feel anxious and frustrated about your ongoing symptoms after varicocele surgery. While some swelling and pain are normal, persistent symptoms should not be ignored. Seeking further evaluation and treatment is essential to ensure proper healing and to address any underlying issues. Remember, your health and comfort are paramount, and you deserve to have your concerns taken seriously by your healthcare providers.
