Right nipple pain?
I have been experiencing pain in my right nipple for over a week.
The right nipple has always been larger than the left, so I often squeeze it out of concern for any potential issues.
Recently, I noticed a small hard lump just below the nipple that I hadn't felt before.
However, when I squeeze it, it seems to become inflamed, and there is no unusual discharge, just a slight watery sensation.
What could this issue be? If I refrain from squeezing it, the pain subsides.
Should I still seek medical evaluation? I'm 19 years old and thought I was past the developmental stage, which makes me quite anxious.
Thank you for your help, doctor.

Dr. Zhou Jiazhen reply Breast and Thyroid
Dear Xiao Huang,
Hello.
The symptoms you described are more consistent with gynecomastia, which may sometimes present with palpable lumps and tenderness similar to breast symptoms experienced during a female menstrual cycle.
It is advisable to monitor the situation, and I recommend following up at a breast surgery outpatient clinic.
Wishing you peace and health.
Sincerely,
Zhou Jiazhen

More Info
Understanding right nipple pain can be concerning, especially when accompanied by the discovery of a small hard lump. It's important to approach this situation with a clear understanding of potential causes and when to seek medical help.
First, let's address the pain and the lump you've noticed. Nipple pain can arise from various factors, including hormonal changes, infections, or benign conditions. Given your age (19), it is less likely that this is related to breast cancer, which is more common in older individuals. However, it is still crucial to consider other possibilities.
1. Hormonal Fluctuations: Hormonal changes, particularly those related to the menstrual cycle, can lead to breast tenderness and changes in the breast tissue. This is often cyclical and may resolve on its own.
2. Infections: Conditions such as mastitis, which is an infection of the breast tissue, can cause pain and swelling. This is more common in breastfeeding women but can occur in others as well. If you notice redness, warmth, or fever, this could indicate an infection that requires medical attention.
3. Benign Conditions: Fibrocystic breast changes are common and can cause lumps and pain. These are usually not serious and can fluctuate with your menstrual cycle.
4. Trauma or Irritation: If you frequently manipulate or squeeze the area, it could lead to irritation or inflammation. This could explain why the pain subsides when you refrain from touching it.
5. Ductal Issues: Sometimes, issues with the milk ducts, such as duct ectasia, can cause pain and discharge. While this is more common in older women, it can occur at any age.
Given that you mentioned the lump is small and you do not see any unusual discharge (other than a little watery fluid), it may not be an immediate cause for alarm. However, the fact that it causes pain when manipulated suggests that it may be inflamed or irritated.
When to Seek Help
You should consider seeking medical advice if:
- The pain persists or worsens over time.
- You notice any changes in the lump, such as an increase in size or hardness.
- There is any unusual discharge from the nipple, especially if it is bloody or has an odor.
- You experience any other symptoms, such as fever, redness, or swelling in the breast.
- You have a family history of breast issues or cancer.
Recommendations
1. Avoid Manipulation: Try to avoid squeezing or manipulating the area, as this can exacerbate inflammation and pain.
2. Monitor Symptoms: Keep track of any changes in the lump or pain. Note if it correlates with your menstrual cycle or any other activities.
3. Consult a Healthcare Provider: If the pain continues or if you have any concerns, it would be wise to consult a healthcare provider. They may perform a physical examination and, if necessary, imaging studies such as an ultrasound to evaluate the lump further.
4. Self-Care: In the meantime, wearing a supportive bra and applying warm compresses may help alleviate discomfort.
In conclusion, while nipple pain and the presence of a lump can be concerning, many causes are benign, especially in younger individuals. However, it is always best to err on the side of caution and consult a healthcare professional if you have any doubts or if symptoms persist. Your health and peace of mind are paramount.
