Laser treatment for onychomycosis?
Hello, doctor.
I have been troubled by the issue of nail fungus for many years.
Due to poor liver function, I am hesitant to take medication.
Recently, I heard that there is a laser treatment available.
Could you please let me know which hospitals in Taiwan offer this procedure? I would greatly appreciate your response.

Hello: The following is transcribed from an article by Dr.
Tseng Chung-Jen.
The FDA in the United States has approved laser treatments for onychomycosis, primarily involving two types of lasers: one is a short-pulse laser at 1064nm or 1320nm, known as the Nd:YAG laser, and the other is a near-infrared dual-wavelength diode laser that operates at two wavelengths (870nm and 930nm).
The former utilizes photothermal principles, while the latter employs photodynamic principles to effectively kill the fungi within the nail.
Importantly, these treatments do not cause damage to the nail bed, nail folds, or the nail itself, providing an alternative option for treating onychomycosis.
In Taiwan, similar laser devices have already been introduced; however, the Department of Health has not yet approved the use of lasers for treating onychomycosis.
Thank you!

Laser treatment for toenail fungus, also known as onychomycosis, has gained popularity as a non-invasive alternative for patients who may be hesitant to use systemic antifungal medications due to concerns about side effects, especially those related to liver function. In Taiwan, while there are various clinics and hospitals that may offer laser treatments for toenail fungus, it is essential to note that the approval and regulation of such treatments can vary.
The two primary types of lasers that have been FDA-approved for treating toenail fungus include the Q-switched Nd:YAG laser (with wavelengths of 1064 nm or 1320 nm) and the dual-wavelength diode laser (with wavelengths of 870 nm and 930 nm). The Nd:YAG laser works on the principle of photothermal action, effectively targeting and destroying the fungal cells within the nail bed without causing damage to the surrounding tissue. The dual-wavelength diode laser, on the other hand, utilizes a different mechanism known as photodynamic action, which also aims to eliminate the fungal infection.
In Taiwan, while there are clinics that have adopted laser technology for treating toenail fungus, it is crucial to verify whether these treatments have received the necessary approvals from the Taiwanese health authorities. As of now, there may not be widespread recognition or endorsement from the Ministry of Health and Welfare regarding laser treatments specifically for toenail fungus. Therefore, patients should consult with dermatologists or podiatrists who are knowledgeable about the latest treatment options and can provide guidance on the most effective and safe methods available.
When considering laser treatment, patients should also be aware of the potential costs involved, as laser therapy can be more expensive than traditional antifungal medications. Additionally, multiple sessions may be required to achieve optimal results, and the effectiveness can vary from person to person. It is also important to maintain realistic expectations, as while laser treatment can significantly improve the condition, it may not guarantee complete eradication of the fungus, especially if the infection has been present for an extended period.
For those with liver function concerns, it is advisable to discuss these issues with a healthcare provider before proceeding with any treatment. A thorough evaluation of the patient's medical history, including liver function tests, can help determine the most appropriate treatment plan. In some cases, a combination of topical antifungal treatments and laser therapy may be recommended to enhance the chances of success while minimizing the risk of systemic side effects.
In summary, while laser treatment for toenail fungus is an emerging option in Taiwan, patients should conduct thorough research and consult with qualified healthcare professionals to explore their options. It is essential to ensure that any treatment chosen is safe, effective, and appropriate for the individual's specific health circumstances.
