Ear pain?
Hello Doctor: Recently, I've been experiencing pain in my ear whenever I touch it due to itching, but there are no visible injuries or trauma.
It also hurts when I use a cotton swab to clean it; in fact, just inserting the cotton swab causes pain.
I've seen a doctor, but it hasn't helped.
What should I do? Thank you...

Ear pain can be a complex issue with various underlying causes, and your symptoms suggest that you may be experiencing a condition related to the ear canal or the structures surrounding it. Based on your description, it seems that you are dealing with discomfort that is exacerbated by touching the ear or using cotton swabs, which can indicate several potential issues.
One common cause of ear pain, especially when there is no visible injury or trauma, is external otitis, also known as swimmer's ear. This condition occurs when the skin of the ear canal becomes inflamed, often due to water exposure, infection, or irritation from foreign objects like cotton swabs. The inflammation can lead to pain, itching, and sensitivity in the ear canal. It's important to note that using cotton swabs can sometimes push wax deeper into the ear or cause irritation, which may worsen the pain.
Another possibility is eustachian tube dysfunction, where the tube that connects the middle ear to the back of the throat does not open properly. This can lead to a feeling of fullness, discomfort, and sometimes pain in the ear, especially when swallowing or yawning. This dysfunction can be caused by allergies, sinus infections, or upper respiratory infections.
Additionally, temporal mandibular joint (TMJ) disorders can also manifest as ear pain. The TMJ is located near the ear, and issues with jaw alignment or inflammation can lead to referred pain in the ear. If you notice that your ear pain is associated with jaw movement or clenching, this could be a contributing factor.
Given that you have seen a doctor without relief, it may be beneficial to seek a second opinion or consult with an ear, nose, and throat (ENT) specialist. They can perform a thorough examination, which may include looking at the ear canal with an otoscope, checking for signs of infection or inflammation, and possibly conducting hearing tests to rule out other issues.
In the meantime, here are some general recommendations that may help alleviate your symptoms:
1. Avoid using cotton swabs: While it may be tempting to clean your ears, this can often lead to more irritation or push wax deeper into the canal.
2. Warm compress: Applying a warm compress to the affected ear may help reduce pain and inflammation.
3. Over-the-counter pain relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or acetaminophen can help manage pain.
4. Stay hydrated: If your symptoms are related to eustachian tube dysfunction, staying hydrated can help thin mucus and promote drainage.
5. Avoid irritants: If allergies are a concern, try to avoid known allergens and consider using antihistamines if appropriate.
6. Monitor for additional symptoms: Keep track of any other symptoms that may develop, such as fever, drainage from the ear, or changes in hearing, and report these to your healthcare provider.
In conclusion, ear pain can arise from various causes, and it is crucial to identify the underlying issue to receive appropriate treatment. If your symptoms persist or worsen, do not hesitate to seek further medical evaluation. Your health and comfort are paramount, especially as you pursue your goal of becoming a pilot, where ear health is particularly important due to changes in pressure during flight.
