Wound on the foreskin?

I went to see a doctor today and have already taken medication and applied ointment! However, I have never experienced these symptoms before; the itching and inflammation of the foreskin only occurred in early December last year and early April this year.
I have not had any issues prior to that.
Could you please explain why this is happening? Also, how should I care for my foreskin after cleaning it in the future?
