Hair Follicle Issues: Are Those White Tips Normal? - Dermatology

Issues related to nipple areola?

Hello Doctor: When my hair or pubic hair falls out, there is often a small white bulb at the end of the hair.
For pubic hair, there is a more rounded white bulb at the end.
Is this the hair bulb? Is this considered normal shedding? If it is the hair bulb, will the hair that has fallen out still regrow? Thank you for your response.

UU, 20~29 year old female. Ask Date: 2013/05/06

Dr. Lin Xinpeng reply Dermatology

Hello: The part of the hair root that is enlarged is called the hair bulb.
The center of the hair bulb has a depressed area that contains a cluster of capillaries and nerve tissue known as the dermal papilla.
The nutrients required for hair growth are absorbed and transported by the blood vessels in this area, serving as the manufacturing site for hair.
The white spots at the tips of your hair are part of the dermal papilla.
Approximately 13% of hair is in the telogen phase, during which the connection between the hair and the surrounding hair follicle gradually loosens, leading to hair shedding from the follicle.
The telogen phase lasts about 2 to 3 months, after which the hair enters the anagen phase to begin the next cycle.
Therefore, if there are no significant changes in the rate of hair growth or shedding, it is considered normal hair loss.
However, if you experience excessive hair loss, it is advisable to consider potential issues with the dermal papilla and seek a hair examination at a major hospital.

When it comes to hair loss and the appearance of white tips on fallen hair, it's essential to understand the underlying biology of hair follicles and the hair growth cycle. The white tips you are observing at the end of your fallen hair are typically referred to as the "hair bulb" or "hair root sheath." This structure is a normal part of the hair follicle and indicates that the hair has shed from its follicle.

In a healthy hair growth cycle, hair goes through three main phases: anagen (growth), catagen (transition), and telogen (resting). During the telogen phase, hair is naturally shed, and the presence of a white bulb at the end of the hair strand is a sign that the hair has completed its life cycle and is ready to fall out. This is a normal process, and it does not necessarily indicate any underlying health issues.

If you notice that the hair is falling out with a white tip, it generally means that the hair follicle is still healthy and capable of producing new hair. However, if you are experiencing excessive hair loss or if the hair follicles appear damaged (for example, if the hair is falling out without the white bulb), it may indicate a problem with the hair follicle itself, such as androgenetic alopecia (male or female pattern baldness), alopecia areata, or other dermatological conditions.

Regarding your question about whether the hair will regrow after shedding, if the hair follicle is healthy and functioning properly, it should be able to produce new hair. However, if there are underlying issues such as hormonal imbalances, nutritional deficiencies, or scalp conditions (like seborrheic dermatitis or psoriasis), these could affect the hair's ability to regrow.

To promote healthy hair growth and address any potential issues, consider the following recommendations:
1. Nutrition: A balanced diet rich in vitamins and minerals is crucial for hair health. Nutrients such as biotin, zinc, iron, and vitamins A, C, D, and E play significant roles in hair growth. Foods like leafy greens, nuts, seeds, fish, and lean proteins can help nourish your hair.

2. Scalp Care: Maintaining a healthy scalp is essential for hair growth. Regularly cleansing your scalp to remove excess oil, dirt, and dead skin cells can help prevent clogged hair follicles. Consider using gentle shampoos and conditioners that suit your hair type.

3. Stress Management: High levels of stress can lead to hair loss. Engaging in stress-reducing activities such as exercise, meditation, or hobbies can help improve your overall well-being and potentially benefit your hair health.

4. Avoiding Harsh Treatments: Limit the use of harsh chemical treatments, excessive heat styling, and tight hairstyles that can cause traction alopecia. Instead, opt for gentle hair care practices.

5. Consult a Dermatologist: If you are concerned about your hair loss or if you notice changes in your hair's texture or thickness, it is advisable to consult a dermatologist. They can perform a thorough examination, possibly including a scalp biopsy or blood tests, to determine the cause of your hair loss and recommend appropriate treatments.

In conclusion, the presence of white tips on fallen hair is generally a normal occurrence and indicates that the hair has shed properly. However, if you have concerns about hair loss or scalp health, seeking professional advice is always a wise step to ensure that any underlying issues are addressed promptly.
