The glare is becoming increasingly severe?
Hello Dr.
Wang, I apologize for bothering you again.
I would like to reiterate my previous question.
I have high myopia of approximately -1100 diopters and astigmatism of -5.02.
Two weeks ago, I went to the hospital for an examination and was diagnosed with a retinal tear, for which I underwent laser treatment.
Now, I am noticing an increasing amount of glare from streetlights.
Initially, I only saw two straight lines on the left and right, but now there are straight lines in all eight directions—up, down, left, right—each appearing as thick lines rather than thin ones, and the halos around the lights have become much larger.
Sometimes, my eyes hurt and I tear up frequently.
I have checked the Amsler grid and it seems normal.
I would like to ask the doctor what might be causing these symptoms.

Dr. Wang Zhishun reply Ophthalmology
Hello, based on the symptoms you described (experiencing glare from streetlights, occasional eye pain, and frequent tearing), a specific diagnosis cannot be made.
However, the eye pain and frequent tearing suggest that there may be something wrong.
It is advisable for you to seek medical attention.

After undergoing retinal laser treatment, experiencing glare and vision issues can be concerning, especially for someone with high myopia like yourself. The symptoms you describe, such as increased glare from lights and the appearance of multiple lines or halos around lights, can be attributed to several factors related to your recent treatment and your underlying eye condition.
Firstly, glare and halos are common visual disturbances that can occur after any type of eye surgery, including retinal laser treatments. This is often due to changes in the cornea or the retina, which can affect how light is processed by the eye. In your case, the increase in glare and the appearance of multiple lines could be related to the healing process of the retina post-laser treatment. The laser creates small scars on the retina, which can sometimes lead to temporary visual disturbances as the eye adjusts to these changes.
Additionally, your high degree of myopia (around -11.00 diopters) can also contribute to visual distortions. High myopia is associated with changes in the structure of the eye, including the retina and the vitreous gel. After laser treatment, these changes can become more pronounced, leading to symptoms like glare and visual distortion. The fact that you are experiencing pain and tearing could indicate that your eyes are reacting to the stress of the treatment or that there might be some irritation or inflammation present.
It is also important to consider the possibility of other complications. For instance, if there is any residual vitreous traction on the retina, it could lead to further visual disturbances. This is particularly relevant in cases of high myopia, where the risk of retinal complications is higher. If you notice any new flashes of light, a sudden increase in floaters, or a shadow in your vision, it is crucial to seek immediate medical attention, as these could be signs of retinal detachment.
Regarding your question about the Amsler grid test, it is a useful tool for detecting macular problems, but it may not capture all potential issues, especially those related to peripheral vision or retinal tears. If you have not already done so, it might be beneficial to undergo a comprehensive eye examination, including a dilated fundus exam, to evaluate the health of your retina and to rule out any complications from the laser treatment.
In terms of management, here are some recommendations:
1. Follow-Up Care: Ensure that you have regular follow-up appointments with your eye care provider. They can monitor your symptoms and the healing process of your retina.
2. Symptom Management: If glare is bothersome, consider using anti-reflective lenses in your glasses, which can help reduce glare from lights. Additionally, wearing sunglasses during the day can help manage light sensitivity.
3. Report New Symptoms: Keep a close watch on any new symptoms, such as increased pain, changes in vision, or new floaters. If these occur, seek immediate medical attention.
4. Rest and Hydration: Ensure that you are giving your eyes adequate rest and staying hydrated, as fatigue and dryness can exacerbate visual disturbances.
5. Discuss with Your Doctor: If your symptoms persist or worsen, discuss the possibility of additional treatments or interventions with your eye care provider. They may consider options such as medications to reduce inflammation or further diagnostic tests to assess the retina's condition.
In conclusion, while glare and vision issues after retinal laser treatment can be concerning, they are not uncommon. However, it is essential to monitor your symptoms closely and maintain open communication with your eye care provider to ensure the best possible outcome for your vision health.
