The glans is red and itchy?

Hello, Director Hsu.
I had sexual intercourse with an unfamiliar woman previously, but I used a condom throughout.
After returning home, I experienced itching on my penis, and the glans was completely red, with an unknown yellow discharge.
When I exposed the glans while showering and washed it with soap, it was uncomfortable, but urination was not painful.
Later, I visited Shudian Clinic, where the doctor diagnosed me with phimosis.
I was relieved, thinking I had a serious sexually transmitted infection.
The doctor recommended circumcision to prevent recurrence and prescribed Diprospan for morning and evening application.
After using it, I healed, but since that incident, during masturbation, if the activity is not very intense, I may experience mild redness the next day.
However, if it is intense, I notice signs of inflammation a few hours later or the next day, though not as severe as before.
Is this a residual effect of balanitis? Is this normal? My glans is not visible when my penis is flaccid, but I can easily retract the foreskin to expose the glans.
When erect, I can see the tip of the glans, but not much of it.
I wash daily, so there is no accumulation of dirt.
I would like to ask you, Director Hsu, if circumcision is necessary in my case.
If I undergo the procedure, will it prevent recurrence? Is there a way to completely eliminate this issue? Thank you for your patience in reading my concerns.

Xiao Chen: Hello, inflammation of the foreskin can sometimes be due to bacterial infections and other pathogens.
However, it can also be caused by irritation from chemical substances, such as soap or other cleaning agents.
Generally, it is recommended to clean the foreskin using minimal cleaning products, only with cold water.
After cleaning, applying petroleum jelly can help maintain the health of the glans.
For further reference, you can look at previous responses I provided to other users regarding related issues.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
Online responses should only serve as a reference; if necessary, it is important to seek medical attention at a healthcare facility.

Understanding the symptoms you are experiencing, such as redness, itching on the glans, and the discharge of yellow fluid, is crucial for determining the underlying cause and appropriate treatment. Based on your description, it seems that you may be dealing with a condition known as balanitis, which is inflammation of the glans (the head of the penis). This condition can be caused by various factors, including infections, irritants, or underlying medical conditions.

1. Causes of Redness and Itching: The redness and itching you are experiencing could be due to several reasons. Since you mentioned having unprotected sexual contact, it is essential to consider sexually transmitted infections (STIs) as a potential cause, even though you used a condom. Condoms can reduce the risk of STIs but do not eliminate it entirely. Other possible causes include fungal infections (like candidiasis), bacterial infections, or irritation from soaps or hygiene products. The yellow discharge you noted could indicate an infection, which is something that should be evaluated by a healthcare provider.

2. Diagnosis and Treatment: It’s good that you consulted a doctor who diagnosed you with a long foreskin (phimosis), which can contribute to hygiene issues and recurrent infections. The recommendation for circumcision is common in such cases, as it can help prevent future occurrences of balanitis by making it easier to maintain proper hygiene. The use of topical medications, such as the one prescribed to you, can help alleviate symptoms during the healing process.

3. Post-Treatment Symptoms: The fact that you are experiencing localized redness after masturbation, especially following vigorous activity, could be a sign of mild irritation or inflammation. This is not uncommon, particularly if the skin is sensitive or if there is any friction involved. While it may not be as severe as your previous symptoms, it indicates that the area is still somewhat reactive.
4. Circumcision Considerations: Regarding your question about whether circumcision is necessary, it is often recommended for individuals with recurrent balanitis or phimosis, as it can significantly reduce the risk of future infections and inflammation. After circumcision, many patients report a decrease in symptoms and a lower likelihood of recurrence. However, it is essential to discuss this thoroughly with your healthcare provider, as they can provide personalized advice based on your specific situation.

5. Preventive Measures: Maintaining good hygiene is crucial. Ensure that you clean the area gently but thoroughly, especially under the foreskin if it is retractable. Avoid using harsh soaps or irritants that could exacerbate the condition. If you notice any new symptoms or if the current symptoms worsen, it is important to seek medical attention promptly.

In conclusion, while your symptoms may be related to balanitis and possibly exacerbated by phimosis, consulting with a urologist for a thorough examination and discussing the option of circumcision could provide a long-term solution. It’s essential to address any concerns you have with your healthcare provider to ensure you receive the best care tailored to your needs.
