Postpartum menstruation
Hello, doctor.
I would like to ask you a question.
I gave birth naturally on February 28 and have been breastfeeding since then, producing about 800-900 ml of milk per day.
I had my first menstrual period on May 1, but the flow was light.
Then on June 1, I noticed a small amount of spotting that lasted for two to three days.
This time, on July 2, I had another light period.
Is this normal? Does this count as having a menstrual period? In the days leading up to June 1 and July 2, I found it difficult to express milk, yet the menstrual flow was so light.
Does this still qualify as a menstrual period?

During the first six months of breastfeeding, menstruation typically does not occur, but occasional light vaginal bleeding may happen, which is considered a normal phenomenon.

Understanding postpartum menstruation while breastfeeding can be quite complex, as various factors influence menstrual cycles during this period. After giving birth, many women experience changes in their menstrual cycles, especially if they are breastfeeding. The hormonal environment during breastfeeding is different from that of non-breastfeeding women, primarily due to the influence of prolactin, a hormone that promotes milk production. Elevated levels of prolactin can suppress ovulation, leading to irregular or absent menstrual cycles.
In your case, since you gave birth on February 28 and are still breastfeeding, it is not uncommon to experience irregular menstrual cycles. The fact that you had a menstrual period on May 1, albeit with a low volume, followed by some spotting on June 1, and then another light period on July 2, suggests that your body is beginning to resume its normal hormonal functions, but it may still be influenced by breastfeeding.
Light menstrual bleeding or spotting can occur for several reasons, including hormonal fluctuations, stress, changes in breastfeeding patterns, or even the body's adjustment to postpartum recovery. It is important to note that while you may be experiencing bleeding, the volume and regularity of your periods can vary significantly during the breastfeeding phase. Some women may have very light periods or even experience amenorrhea (absence of menstruation) while exclusively breastfeeding.
Regarding your concerns about the correlation between breastfeeding and menstrual flow, it is essential to understand that the amount of milk you produce can be affected by hormonal changes associated with your menstrual cycle. Many women notice a decrease in milk supply just before their period starts, which could explain why you found it challenging to express milk in the days leading up to your light bleeding.
To answer your question, yes, what you are experiencing can be considered a menstrual cycle, even if the flow is light. It is not unusual for postpartum menstruation to be irregular and vary in volume during the breastfeeding period. However, if you notice any significant changes, such as an increase in pain, a sudden change in the pattern of bleeding, or if you have concerns about your milk supply, it would be prudent to consult with a healthcare provider. They can assess your situation more thoroughly and provide personalized advice.
In summary, postpartum menstruation while breastfeeding can be irregular and light, influenced by hormonal changes and breastfeeding practices. Your experiences of light bleeding and difficulty expressing milk are not uncommon. If you have further concerns or if your symptoms change, seeking medical advice is always a good step to ensure your health and well-being.
