Square face
Hello: I have a large bone structure and a wide face...
My face shape is broader than average, and it feels square.
I read previous medical responses stating that the width of the face cannot be changed.
I would like to know if reducing the cheekbones and shaving the jaw angle can decrease the width of my face? Which hospitals in Taipei have experience with facial bone contouring surgeries? Thank you!

Dr. Jiang Baicong reply Plastic Surgery
Ms.
Jun: Regarding square-shaped faces, they are often characterized by a prominent mandibular angle or an angle of the mandible that is too close to 90 degrees.
The assessment requires examination, such as X-ray imaging to measure the angles of the cranial bones, to understand the relationship between the bones and teeth.
After this evaluation, we can determine if the condition is due to hypertrophy of the masticatory muscles.
If it is muscle hypertrophy, then addressing the muscles will suffice.
If the mandibular angle is prominent without any dental issues, a portion of the protruding mandibular angle can be reduced.
In cases where the mandibular angle is prominent or the angle is too close to 90 degrees, and there are concurrent dental and occlusal issues, it is essential to plan how the teeth and occlusion should be positioned post-surgery before proceeding with mandibular surgery.
As for zygomatic reduction, the surgery involves reducing the zygomatic bone, with primary attention to the location of the incision and the potential impact on the facial nerve.
Hospitals in Taipei such as Mackay Memorial Hospital, Shin Kong Wu Ho-Su Memorial Hospital, Chang Gung Memorial Hospital, Veterans General Hospital, and Tri-Service General Hospital have experience in these procedures, particularly Chang Gung.
Wishing you good health, Jiang Baicong, Qishan Hospital, May 27, 2002.

More Info
Jaw and cheekbone surgery, often referred to as facial contouring or bone reduction surgery, can indeed help slim down a wide face. This type of surgery typically involves reducing the size of the cheekbones (zygomatic bones) and the jawline (mandible) to create a more oval or heart-shaped facial appearance. Many individuals seek this procedure to achieve a more aesthetically pleasing facial structure, especially if they feel their face is too wide or square.
Understanding the Procedure
The surgery can be performed through various techniques, including intraoral (inside the mouth) or extraoral (external incisions) approaches. The surgeon will carefully remove or reshape the bone to achieve the desired contour. For cheekbone reduction, the surgeon may remove a portion of the zygomatic bone, while for jaw reduction, the angle of the mandible can be shaved down to create a softer appearance.
Risks and Considerations
While the surgery can yield significant aesthetic improvements, it is essential to understand the potential risks involved. These may include:
1. Nerve Damage: As mentioned in previous discussions, there is a risk of damaging the facial nerves, particularly the branches of the trigeminal nerve responsible for sensation in the face. This could lead to temporary or, in rare cases, permanent numbness or altered sensation.
2. Infection: As with any surgical procedure, there is a risk of infection, which can complicate recovery and affect the final results.
3. Scarring: Depending on the surgical approach, there may be visible scars, although skilled surgeons aim to minimize this.
4. Asymmetry: There is a possibility of asymmetrical results, which may require additional procedures to correct.
5. Changes in Bite: Altering the jaw structure can affect how the teeth align, potentially leading to bite issues.
Probability of Complications
The likelihood of experiencing severe complications, such as permanent nerve damage, is relatively low, especially when the procedure is performed by an experienced and qualified surgeon. Most studies suggest that the risk of significant nerve injury is less than 5% in skilled hands. However, it is crucial to have a thorough pre-operative consultation where you can discuss your concerns and the surgeon can evaluate your specific anatomy.
Reducing Risks
To minimize the risks associated with jaw and cheekbone surgery, consider the following steps:
1. Choose a Qualified Surgeon: Look for a board-certified plastic surgeon or oral and maxillofacial surgeon with extensive experience in facial contouring procedures. Research their credentials, read patient reviews, and ask to see before-and-after photos of previous patients.
2. Pre-operative Assessment: Ensure a comprehensive evaluation of your facial structure and medical history. This will help the surgeon tailor the procedure to your needs and identify any potential complications.
3. Follow Post-operative Care Instructions: Adhering to your surgeon's post-operative care guidelines is crucial for a smooth recovery and optimal results.
4. Discuss Anesthesia Options: Depending on the extent of the surgery, different anesthesia options may be available. Discuss these with your surgeon to ensure you are comfortable.
Finding a Surgeon in Taipei
In Taipei, several hospitals and clinics specialize in facial plastic surgery. Look for institutions with a strong reputation in cosmetic surgery, such as Taipei Medical University Hospital or Chang Gung Memorial Hospital. It is advisable to schedule consultations with multiple surgeons to find one whose approach and philosophy align with your expectations.
Conclusion
In summary, jaw and cheekbone surgery can effectively slim down a wide face, but it is essential to weigh the benefits against the risks. By choosing a qualified surgeon and following proper pre- and post-operative care, you can enhance your chances of achieving a successful outcome. If you have further questions or concerns, do not hesitate to reach out to a medical professional for personalized advice.
