Regarding medication issues?
Hello Dr.
Tsai,
A friend gave me Silymarin 150mg capsules for liver protection, and I have been taking one capsule daily for about 100 days.
Recently, I noticed small red spots on my hands and feet that are swollen and itchy, which feels like an allergic reaction (I do not have any known allergies).
Could this be a side effect of the medication, and should I stop taking it? Thank you.

Hello: Common side effects of silymarin include gastrointestinal issues such as bloating, indigestion, diarrhea, as well as headaches and dizziness.
At high doses, it may cause a laxative effect.
Your symptoms are relatively uncommon; if you suspect side effects, you should discontinue use immediately and seek medical attention.

More Info
Silymarin, derived from the milk thistle plant, is commonly used as a dietary supplement for liver health. While it is generally considered safe for most people, like any medication or supplement, it can have side effects. The symptoms you describe—red spots, swelling, and itching—could indeed be indicative of an allergic reaction or an adverse effect related to the use of Silymarin, especially since you mentioned that these symptoms appeared after approximately 100 days of consistent use.
Common side effects of Silymarin include gastrointestinal issues such as bloating, indigestion, and diarrhea, as well as headaches and dizziness. However, the symptoms you are experiencing are less commonly reported. Allergic reactions can manifest in various ways, including skin rashes, hives, or other dermatological symptoms. It is important to note that even if you do not have a known history of allergies, it is still possible to develop an allergic reaction to a substance after prolonged exposure.
Given the nature of your symptoms, it is advisable to discontinue the use of Silymarin and consult a healthcare professional as soon as possible. They can evaluate your condition, possibly perform allergy testing, and determine whether your symptoms are indeed related to the supplement or if they may be due to another underlying issue. It is crucial to address these symptoms promptly, especially if they worsen or if you experience additional symptoms such as difficulty breathing, swelling of the face or throat, or severe dizziness, which could indicate a more serious allergic reaction.
In addition to stopping the supplement, your healthcare provider may recommend antihistamines or other treatments to alleviate your symptoms. They may also suggest alternative liver-supporting supplements or lifestyle changes that could be beneficial without the risk of adverse reactions.
Furthermore, it is essential to keep in mind that dietary supplements are not regulated as strictly as prescription medications, which means that the quality and concentration of active ingredients can vary significantly between products. This variability can sometimes lead to unexpected side effects. Always consult with a healthcare provider before starting any new supplement, especially if you have pre-existing health conditions or are taking other medications.
In summary, while Silymarin is generally safe for liver health, your symptoms of red spots and itching could be a sign of an adverse reaction. It is best to stop taking the supplement and seek medical advice to ensure your health and safety.
