Respiratory lung vibrations

Hello Doctor: Recently, I have been feeling a vibration in a spot just below my right armpit when I breathe.
When I place my hand there, I can definitely feel the vibration as well.
However, it is intermittent; sometimes it occurs, and sometimes it does not.
What could be the possible issue? Thank you for your clarification.

A Bin, 10~19 year old female. Ask Date: 2013/08/09

Dr. Huang Yiwen reply Pulmonology

Dear Ah Bin: The lungs are an organ of respiration that typically does not have sensory feedback, and there is usually no vibratory response during normal breathing.
Your description seems to indicate muscle twitching, which could be related to localized nerve conduction issues or possibly an electrolyte imbalance.
Additionally, an important consideration in the axillary region is the lymphatic system.
Therefore, it is essential for you to visit a hospital for a physician to evaluate your concerns.
I hope this information is helpful to you.
Sincerely, Huang Yi-Wen

Intermittent chest vibrations, particularly localized to a specific area such as under the right armpit, can be concerning and may have several potential causes. Understanding these vibrations requires a comprehensive approach, considering various factors including the patient's medical history, physical examination, and possibly further diagnostic testing.

1. Musculoskeletal Causes: One of the most common reasons for localized vibrations or tremors in the chest area is related to musculoskeletal issues. This could include muscle spasms or contractions in the intercostal muscles (the muscles between the ribs) or pectoral muscles. These spasms can occur due to overuse, strain, or even stress. If you've recently engaged in physical activity or have been under stress, this could be a contributing factor.

2. Nerve Irritation or Compression: Another possibility is irritation or compression of the nerves that supply the area. Conditions such as thoracic outlet syndrome, where nerves or blood vessels are compressed in the space between the collarbone and the first rib, can lead to sensations of vibration or tingling. Similarly, a pinched nerve in the cervical spine can cause referred sensations in the chest area.

3. Respiratory Issues: Although less common, respiratory conditions could also cause sensations of vibration. For example, if there is an underlying condition such as asthma or bronchitis, the associated coughing or breathing difficulties might lead to muscle contractions that could be perceived as vibrations. Additionally, conditions like pleurisy, which involves inflammation of the lining of the lungs, could also lead to discomfort or unusual sensations in the chest area.

4. Cardiac Considerations: While less likely given the description, it is important to consider cardiac issues, especially if there are accompanying symptoms such as chest pain, shortness of breath, or palpitations. Arrhythmias or other heart conditions can sometimes present with unusual sensations in the chest. However, if the vibrations are isolated and not associated with other symptoms, this is less likely.

5. Psychogenic Factors: Anxiety and stress can manifest physically in various ways, including muscle tension and spasms. If you are experiencing increased stress or anxiety, this could potentially contribute to the sensations you are feeling.

6. Other Considerations: Other less common causes could include conditions such as fibromyalgia, which can cause widespread pain and unusual sensations, or even localized infections or inflammation in the chest wall.

Given the intermittent nature of the vibrations and the fact that they are localized, it is advisable to monitor the situation. If the vibrations persist, worsen, or are accompanied by other concerning symptoms (such as pain, difficulty breathing, or changes in heart rate), it would be prudent to seek medical evaluation. A healthcare provider may perform a physical examination and consider imaging studies, such as a chest X-ray or MRI, to rule out any underlying structural issues.
In conclusion, while intermittent chest vibrations can have several benign causes, it is essential to remain vigilant and consult a healthcare professional if there are any changes or concerns. Early evaluation can help ensure that any potential issues are addressed promptly and appropriately.
