There is blood-tinged discharge during breastfeeding?
Hello Doctor: I gave birth at the end of May, and it has been almost three months since then.
I am still exclusively breastfeeding.
Recently, I have noticed some slight blood-tinged discharge from my vagina (once a day), but it doesn't seem like a menstrual period.
Is this normal? Thank you!
Coco, 30~39 year old female. Ask Date: 2013/08/17
Dr. Lin Wenbin reply Obstetrics and Gynecology
It is recommended to come in for an outpatient examination, as there could be many possible conditions.
Even if it is the first menstrual period, it may differ from the usual pattern of menstruation.

Blood-streaked discharge during breastfeeding can be concerning, but it is important to understand the context and potential causes. After childbirth, many women experience a variety of changes in their bodies, including hormonal fluctuations and physical adjustments as they begin breastfeeding.
Firstly, it is essential to differentiate between normal postpartum discharge (lochia) and any abnormal bleeding. Lochia is the vaginal discharge that occurs after childbirth, consisting of blood, mucus, and uterine tissue. It typically changes in color and consistency over the weeks following delivery, starting as bright red blood and gradually transitioning to pink or brown before eventually becoming yellowish-white. This discharge can last for several weeks, and it is not uncommon for women to notice occasional blood streaks during this time.
However, if you are experiencing blood-streaked discharge that is not consistent with the typical lochia, it may warrant further investigation. Here are some potential causes for blood-streaked discharge while breastfeeding:
1. Hormonal Changes: After childbirth, your body undergoes significant hormonal changes as it adjusts to breastfeeding and the postpartum period. These hormonal fluctuations can sometimes lead to irregular bleeding or spotting, which may appear as blood-streaked discharge.
2. Nipple Trauma or Infection: If you are breastfeeding, it is possible that you may have experienced some trauma to the nipples, which can lead to localized bleeding. Additionally, infections such as mastitis can cause symptoms like redness, swelling, and discharge from the nipple, which may include blood.
3. Uterine Involution: The uterus undergoes a process called involution after childbirth, where it contracts and returns to its pre-pregnancy size. This process can sometimes cause cramping and minor bleeding, which may be reflected in vaginal discharge.
4. Retained Products of Conception: In some cases, if there are retained placental fragments or other products of conception in the uterus, this can lead to abnormal bleeding. This situation is less common but should be evaluated if the bleeding persists or is accompanied by other concerning symptoms.
5. Other Medical Conditions: Conditions such as fibroids, polyps, or other gynecological issues can also lead to abnormal bleeding. If the blood-streaked discharge continues or is accompanied by pain, fever, or other unusual symptoms, it is crucial to seek medical attention.
In conclusion, while some blood-streaked discharge can be normal during the postpartum period, especially while breastfeeding, it is essential to monitor the situation closely. If the discharge persists, increases in volume, or is accompanied by other symptoms such as pain or fever, it is advisable to consult with a healthcare provider for a thorough evaluation. They may perform a physical examination and possibly imaging studies to rule out any underlying issues. Your health and well-being are paramount, and any concerns should be addressed promptly to ensure a safe and healthy postpartum experience.
