Does varicocele on one side affect the other side?
Hello Dr.
Hsu,
At the end of June, I was diagnosed with left-sided varicocele and underwent a left-sided varicocelectomy in early July.
Since the surgery, the discomfort on the left side has improved significantly; however, I have started to experience discomfort on the right side.
It is not extremely painful, but I can definitely feel it.
This sensation extends to my right lower back, where I feel a tight and constricted sensation.
I am unsure if this is related to the left side not being fully healed yet.
When I returned for a follow-up with my original physician, they suggested that it might be a psychological issue and advised me to observe the situation further.
The ultrasound performed at the time of my diagnosis (at the end of June) showed no issues on the right side.
I am concerned that this might indicate a simultaneous occurrence on both sides.
Could you please clarify this for me? Thank you!

Hello, generally speaking, the likelihood of bilateral varicocele is quite low.
If the pain persists, a follow-up ultrasound of the scrotum can be considered.
If there are any unclear aspects, feel free to ask again, and we can discuss and research further.

The question regarding whether left varicocele surgery can affect symptoms on the right side is quite complex and involves understanding the anatomy and physiology of the male reproductive system, particularly the venous drainage of the testes.
Varicocele is a condition characterized by the enlargement of the veins within the scrotum, specifically the pampiniform plexus, which can lead to discomfort, pain, and even fertility issues. It is most commonly found on the left side due to anatomical differences in venous drainage. The left testicular vein drains into the left renal vein, while the right testicular vein drains directly into the inferior vena cava. This difference in drainage can lead to a higher incidence of varicocele on the left side.
In your case, after undergoing surgery for left varicocele, you have noticed discomfort on the right side. It is important to consider several factors:
1. Post-Surgical Recovery: After surgery, it is common for patients to experience discomfort or pain in areas adjacent to the surgical site. This can be due to inflammation, nerve irritation, or referred pain. The body may take time to heal, and sensations can sometimes be misinterpreted as new or different issues.
2. Psychological Factors: As your physician mentioned, there can be a psychological component to postoperative pain. Anxiety about the surgery and the potential for complications can lead to heightened awareness of bodily sensations, which may be perceived as discomfort or pain.
3. Potential for Concurrent Issues: While it is less common, it is possible for varicocele to develop on the right side, or for other conditions to manifest that could cause similar symptoms. If the ultrasound indicated no issues on the right side prior to surgery, it is less likely that a varicocele is the cause of your current discomfort. However, it is still advisable to monitor the situation and consult with your physician if symptoms persist.
4. Referred Pain: The anatomy of the male reproductive system means that pain can sometimes be referred from one area to another. The discomfort you feel on the right side and into the right flank could be related to the healing process of the left side or even unrelated issues such as muscle strain or nerve irritation.
5. Follow-Up Care: It is crucial to maintain follow-up appointments with your healthcare provider. If your symptoms do not improve or worsen, further imaging studies or evaluations may be warranted to rule out any complications or new developments.
In conclusion, while it is unlikely that the surgery for left varicocele directly caused symptoms on the right side, the interplay of healing, psychological factors, and the potential for other conditions should be considered. It is essential to communicate openly with your healthcare provider about your symptoms and concerns, as they can provide the most accurate assessment and recommendations based on your specific situation. If necessary, further diagnostic imaging or evaluations may be needed to ensure that there are no underlying issues contributing to your discomfort.
