Genital bleeding

Hello doctor, my menstrual period started on August 17 and had a break, then I experienced bleeding again on the evening of August 30.
On that day, I felt pain and soreness in my lower back.
In the past, the pain has been so severe that I couldn't stand and just wanted to lie down, and I would also use heat packs on my lower back.
This happens every month after my period, and I always experience bleeding.
I have seen a doctor, but my condition hasn't improved; I've undergone urine tests that indicated an infection.
I've been trying to drink mineral water and plain water, but the situation remains the same.
I've also been advised to take cranberry supplements, but it hasn't helped either.
I'm unsure what to think; could this be a kidney issue due to the recurrent infections, or is it related to my uterine fibroids? I have uterine fibroids, but the bleeding doesn't seem typical.
My symptoms include vaginal bleeding, itching in the vaginal area, and pain throughout my lower back.

More Info

Abnormal vaginal bleeding can be a distressing experience for many women, and understanding its potential causes and solutions is crucial for managing one's health effectively. Based on your description, it appears that you are experiencing irregular bleeding, accompanied by discomfort in the lower back and potential itching in the genital area. Let's break down the possible causes and solutions for your symptoms.

Possible Causes of Abnormal Vaginal Bleeding
1. Hormonal Imbalances: Hormonal fluctuations can lead to irregular bleeding. Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can disrupt the normal menstrual cycle, resulting in unexpected bleeding.

2. Uterine Fibroids: You mentioned having uterine fibroids, which are non-cancerous growths in the uterus. Fibroids can cause heavy menstrual bleeding, prolonged periods, and spotting between periods. They can also contribute to pelvic pain and discomfort.

3. Infections: Urinary tract infections (UTIs) or sexually transmitted infections (STIs) can lead to abnormal bleeding. If you have been experiencing recurrent UTIs, this could be a contributing factor. Infections can also cause itching and discomfort in the genital area.

4. Endometriosis: This condition occurs when tissue similar to the lining of the uterus grows outside the uterus, leading to pain and irregular bleeding. It can cause significant discomfort, especially during menstruation.

5. Cervical or Uterine Polyps: These are benign growths on the cervix or inside the uterus that can cause abnormal bleeding. They are often asymptomatic but can lead to spotting or bleeding between periods.

6. Pregnancy-Related Issues: Although you may have ruled out pregnancy, it's important to consider that conditions such as ectopic pregnancy or miscarriage can also present with abnormal bleeding.

Solutions and Recommendations
1. Consult a Specialist: Given your ongoing symptoms and the fact that you have already seen a doctor, it may be beneficial to seek a second opinion from a gynecologist who specializes in abnormal bleeding. They may recommend further diagnostic tests, such as a pelvic ultrasound or hysteroscopy, to assess the condition of your uterus and ovaries.

2. Monitor Symptoms: Keep a detailed record of your menstrual cycle, including the timing, duration, and characteristics of your bleeding, as well as any associated symptoms like pain or itching. This information can help your healthcare provider make a more accurate diagnosis.

3. Lifestyle Modifications: Staying hydrated and maintaining a balanced diet can support overall health. If you are experiencing recurrent UTIs, consider discussing preventive measures with your doctor, such as dietary changes or supplements like cranberry extract.

4. Medication Management: If hormonal imbalances are suspected, your doctor may recommend hormonal treatments such as birth control pills to help regulate your cycle and reduce bleeding. If fibroids are the cause, treatments may include medication or surgical options, depending on their size and symptoms.

5. Pain Management: For the discomfort in your lower back, over-the-counter pain relievers such as ibuprofen or naproxen may help alleviate pain. However, consult your healthcare provider before starting any new medication.

6. Addressing Infections: If infections are a concern, your doctor may prescribe antibiotics or antifungal medications to treat any underlying issues. It's essential to complete the full course of treatment as prescribed.

Conclusion
Abnormal vaginal bleeding can stem from various causes, and it is essential to work closely with your healthcare provider to identify the underlying issue. Given your history of uterine fibroids and recurrent infections, a thorough evaluation is warranted to determine the best course of action. By actively participating in your healthcare and seeking appropriate treatment, you can manage your symptoms effectively and improve your quality of life. Remember, open communication with your healthcare provider about your symptoms and concerns is key to finding the right solutions.
