Inward-growing eyelashes?

Hello, doctor.
I would like to ask about a condition I experience with my eyes.
Every day, around 4 to 5 PM, I feel as if sand has flown into my eyes.
It happens consistently at that time, and wiping my eyes with a tissue seems to help.
What could this condition be? Is it possible that I have ingrown eyelashes? Why does it occur at the same time every day? How can I resolve this issue? Thank you, doctor!

Chen Xiaoguang, 20~29 year old female. Ask Date: 2013/10/17

Dr. Gao Pilin reply Ophthalmology

Hello, Xiaoguang: Based on your description, since it only occurs in the afternoon, it is unlikely to be due to ingrown eyelashes, as that would typically cause persistent discomfort rather than just in the afternoon.
The fact that it happens at a specific time in the afternoon may indicate issues such as eye fatigue or dryness.
A personal examination is necessary for further confirmation.
The ophthalmology department at Puzi Hospital is open Monday to Friday from 8:00 AM to 12:00 PM and Wednesday afternoons from 1:00 PM to 4:30 PM.
I hope to have the opportunity to assist you.

Eyelash inversion, also known as trichiasis, occurs when the eyelashes grow inward toward the eye instead of outward. This condition can lead to significant discomfort, as the eyelashes can irritate the cornea and conjunctiva, causing symptoms such as a gritty sensation, redness, tearing, and even pain. Your description of feeling as if sand is flying into your eye around the same time each day suggests that there may be an underlying issue that could be related to eyelash inversion or another condition affecting your eyelids or tear film.

There are several potential causes for your symptoms. First, it is essential to consider the possibility of eyelash inversion. If your eyelashes are indeed growing inward, they can continuously irritate your eye, leading to the sensation you described. Additionally, other factors such as dry eyes, allergies, or environmental irritants could contribute to your discomfort. The fact that you experience this sensation at a specific time each day may indicate that certain activities or environmental conditions (like exposure to air conditioning or dust) exacerbate your symptoms.

To address your concerns, it is crucial to consult with an eye care professional, such as an ophthalmologist or optometrist. They can perform a thorough examination of your eyelids and eyelashes to determine if trichiasis is present. If eyelash inversion is confirmed, treatment options may include:
1. Eyelash Removal: In some cases, temporarily removing the offending eyelashes can provide immediate relief. This can be done in the office by a trained professional.

2. Electrolysis or Laser Treatment: For a more permanent solution, electrolysis or laser treatment can be used to destroy the hair follicles of the misaligned eyelashes, preventing them from growing back inward.

3. Lubricating Eye Drops: If your symptoms are exacerbated by dry eyes, using artificial tears or lubricating eye drops can help alleviate discomfort.

4. Managing Environmental Factors: If your symptoms are related to environmental irritants, consider using protective eyewear when outdoors or in dusty environments, and ensure that your living space is free from allergens.

5. Addressing Underlying Conditions: If your eye discomfort is related to dry eyes or allergies, treating these underlying conditions can significantly improve your symptoms.

In addition to eyelash inversion, other conditions such as blepharitis (inflammation of the eyelid margins), meibomian gland dysfunction, or even conjunctivitis could also cause similar symptoms. Therefore, a comprehensive evaluation is essential to determine the exact cause of your discomfort.

In summary, while eyelash inversion could be a potential cause of your symptoms, it is crucial to seek professional evaluation to rule out other conditions and receive appropriate treatment. Regular eye examinations and maintaining good eyelid hygiene can also help prevent and manage discomfort. If you notice any changes in your symptoms or if they worsen, do not hesitate to seek medical attention promptly.
