Is it adult phimosis or excessive foreskin?
Hello Doctor, I would like to ask the following questions:
1.
If the glans is completely covered by the foreskin when not erect, and there is about 1 cm of foreskin that does not cover the penis (meaning the foreskin length is longer than the penis), but I can freely push it down to expose the glans; when erect, if it is not very hard, I can see the urethral opening; but when it is harder or fully erect, the foreskin automatically retracts to halfway down the glans or completely retracts to expose the glans, and the foreskin can also slide freely during erection without any discomfort, is there a need for circumcision?
2.
Does my situation qualify as phimosis or simply a long foreskin?
3.
Is it harmful to the skin of the glans to wash it with body wash over a long period?
4.
When erect, since the foreskin can easily slide back and expose the glans, I have the habit of retracting the foreskin.
Will this have any adverse effects on blood circulation? (I usually keep it retracted for about 10 to 20 minutes)
I would appreciate your help in answering these questions! Thank you!

Hello: 1.
Based on your description, your foreskin is only slightly elongated and there is no issue of phimosis, so circumcision is not necessary.
2.
It is simply a case of excessive foreskin.
3.
Cleaning is certainly important and should be done; it will not harm the skin of the glans, so you can rest assured.
4.
Fully retracting the foreskin is a normal action and will not adversely affect blood circulation.
Wishing you good health.

When it comes to understanding conditions like phimosis or excessive foreskin, it's important to consider both the physical aspects and the potential implications for health and hygiene. Let's address your questions one by one.
1. Do you need a circumcision? Based on your description, it seems that you do not have a severe case of phimosis. If you can retract the foreskin freely when not erect and it does not cause discomfort during erection, then circumcision may not be necessary. Circumcision is typically recommended when there is a significant inability to retract the foreskin, leading to pain, recurrent infections, or other complications. Since you mentioned that the foreskin can slide back and forth without discomfort, it suggests that your condition may not warrant surgical intervention.
2. Is your condition considered excessive foreskin or adult phimosis? From your description, it sounds more like you have excessive foreskin rather than true phimosis. Phimosis is characterized by a tight foreskin that cannot be retracted over the glans (the head of the penis), especially when erect. In your case, since you can retract the foreskin without discomfort, it indicates that you do not have phimosis, but rather a longer than average foreskin.
3. Is using body wash harmful to the glans? Using body wash or soap to clean the glans can be acceptable, but it is crucial to choose a mild, unscented product. The skin on the glans is sensitive, and harsh soaps can lead to irritation or dryness. It is generally recommended to clean the area with warm water and a gentle cleanser, avoiding anything that may cause irritation. If you notice any redness, itching, or discomfort, it may be wise to switch to a more suitable product or consult a healthcare provider.
4. Does retracting the foreskin affect blood circulation? Retracting the foreskin for short periods (10-20 minutes) should not cause any significant issues with blood circulation, especially if you are comfortable and do not experience any pain or numbness. However, it is essential to ensure that the foreskin is not retracted too tightly, as this could lead to constriction and potential complications. If you notice any unusual symptoms such as swelling, pain, or discoloration, it would be advisable to consult a healthcare professional.
In summary, based on your description, it appears that you have excessive foreskin rather than phimosis, and circumcision may not be necessary at this time. Maintaining good hygiene with gentle products is important, and retracting the foreskin for short periods should not pose a risk to blood circulation as long as it is done comfortably. If you have ongoing concerns or notice any changes, it would be best to consult a urologist or healthcare provider for personalized advice and assessment.
