Unusual Sensations in the Anus: Seeking Solutions for Persistent Discomfort - Surgery

There is a strange sensation in the anus?

Hello Doctor: I have a feeling of looseness in my anus, as well as a sensation of pressure.
When I squat and then stand up, or when I jump, I experience this looseness.
Bending over to lift heavy objects causes a feeling of pressure.
If I exert too much during a bowel movement or after jogging, I experience significant pain in the groove on the right side of my anus extending to the right side of my coccyx.
However, after visiting the surgeon, they said there is nothing wrong with my anus and advised me to soak in warm water and take anti-inflammatory medication, but I have not seen any improvement after 7 months.
Doctor, are there any other options available to me? Thank you.

Dr. Xu Kaixi reply Surgery

Hello, if you have undergone an examination in the surgical department and there are no issues with the anus, the symptoms you described may be related to a lumbar disc herniation.
It is recommended that you visit the neurosurgery department for further evaluation.

It sounds like you're experiencing some persistent and uncomfortable sensations in the anal area, which can be quite distressing. Based on your description, it seems that you are dealing with a combination of sensations, including a feeling of looseness, pressure, and pain, particularly after certain activities like squatting, lifting heavy objects, or during bowel movements.
First and foremost, it's important to understand that while you have already consulted with a surgeon and received some advice, the absence of findings during examinations does not necessarily mean that there is no underlying issue. Sometimes, conditions affecting the anal region may not be easily visible during a physical examination, and further investigation may be warranted.

Here are several potential avenues to consider:
1. Pelvic Floor Dysfunction: The symptoms you describe could be related to pelvic floor dysfunction. This condition can lead to issues with the muscles that support the pelvic organs, including the rectum. A physical therapist who specializes in pelvic floor therapy can help assess your situation and provide exercises to strengthen or relax these muscles.

2. Anal Fissures or Hemorrhoids: Even if a physical examination did not reveal any issues, it might be worth considering the possibility of anal fissures or hemorrhoids. These can sometimes be subtle and may not be easily detected. If you experience pain during bowel movements, this could be a sign of fissures. Over-the-counter treatments, dietary changes to ensure softer stools, and proper hydration can help alleviate these conditions.

3. Dietary Adjustments: Ensuring that you have a high-fiber diet can help prevent constipation and reduce straining during bowel movements, which may alleviate some of the pressure and discomfort you are feeling. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Additionally, staying well-hydrated is crucial.

4. Stress and Anxiety Management: Sometimes, stress and anxiety can manifest physically, leading to tension in the pelvic area. Techniques such as mindfulness, yoga, or meditation can help reduce overall tension and may alleviate some of the sensations you are experiencing.

5. Consult a Specialist: If your symptoms persist despite conservative measures, it may be beneficial to seek a second opinion from a gastroenterologist or a colorectal specialist. They may recommend further diagnostic tests, such as an MRI or a colonoscopy, to rule out any underlying conditions that may not have been detected in previous examinations.

6. Medication: If inflammation is suspected, and over-the-counter anti-inflammatory medications have not provided relief, discussing prescription options with your healthcare provider may be worthwhile. Additionally, topical treatments specifically designed for anal discomfort may provide some relief.

7. Sitz Baths: Continuing with sitz baths can be beneficial. Soaking in warm water can help relax the muscles in the anal area and promote healing if there are any minor irritations.

8. Avoiding Strain: When using the bathroom, try to avoid straining. If you find that you are having difficulty, consider using a stool to elevate your feet, which can help create a more natural position for bowel movements.

In conclusion, while it can be frustrating to deal with ongoing discomfort without a clear diagnosis, there are multiple approaches you can take to address your symptoms. It may take some time and experimentation to find the right combination of treatments that work for you. Don't hesitate to advocate for yourself and seek further medical advice if your symptoms do not improve. Your comfort and well-being are important, and there are healthcare professionals who can help you navigate this issue.
