Is This Diarrhea? Pediatric Bowel Changes After Illness - Pediatrics

Is this considered diarrhea?

Hello Dr.
Hsiao: My younger brother had a cold last week and was taking medication for it.
During that time, I noticed that before he had a bowel movement, there would be some mucus-like discharge with a stool color, which was in clumps.
I thought this was caused by the cold medication.
Now, his cold has improved, and he has stopped taking the medication for about three days.
However, I noticed that today, before he had a bowel movement, he was passing some clear liquid from his anus, along with some mucus-like discharge that also had a stool color, and it was similarly in clumps! Sometimes, this occurs all at once with one bowel movement, while other times it takes two to three bowel movements to fully pass.
However, the stool itself is hard and tubular in nature.
What could be going on? Is this considered diarrhea? Aside from these symptoms, my younger brother's overall energy and well-being are good!

Dr. Xiao Kaiwen reply Pediatrics

This condition is commonly referred to as "overflow diarrhea," which usually occurs when hard stools become impacted at the anal opening, allowing only small amounts of liquid stool to seep out.
Therefore, this is not diarrhea; rather, it may indicate hard stools or even constipation.

When it comes to understanding bowel changes in children, especially after an illness, it's important to consider several factors, including the nature of the stool, the presence of mucus, and the overall health of the child. Based on your description, your brother has experienced some changes in his bowel movements following a recent illness, which is not uncommon.

Firstly, the presence of mucus in the stool can be a normal occurrence, especially after gastrointestinal disturbances. Mucus is produced by the intestines and can sometimes be seen in the stool when the gut is irritated or inflamed. This can happen after a viral infection, such as a cold, which may not directly affect the gastrointestinal tract but can lead to changes in bowel habits due to stress, dietary changes, or the effects of medication.

The fact that your brother's stool is described as "hard and tubular" suggests that he is not experiencing diarrhea in the traditional sense, which is typically characterized by loose, watery stools. However, the intermittent presence of clear liquid and mucus could indicate that his intestines are still adjusting after the illness. This can sometimes lead to a phenomenon known as "overflow diarrhea," where liquid stool leaks around a hard stool mass, resulting in the appearance of diarrhea.

It's also worth noting that the consistency of stool can be influenced by hydration levels, diet, and the body's response to any medications taken during the illness. If your brother has been less active or has had a reduced appetite, this could also contribute to harder stools.
In terms of whether this constitutes diarrhea, it seems that your brother is not experiencing diarrhea per se, but rather a combination of normal stool and some mucus discharge, which may be a residual effect of his recent illness. Since you mentioned that he is otherwise in good spirits and has a good energy level, this is a positive sign.

However, it is essential to monitor his symptoms closely. If the mucus continues to be present in significant amounts, if he develops any additional symptoms such as abdominal pain, fever, or if the stool consistency changes to become more liquid, it would be advisable to consult a pediatrician. They may recommend further evaluation to rule out any underlying gastrointestinal issues or infections.

In conclusion, while the changes in your brother's bowel movements may be concerning, they are not uncommon after an illness. The presence of mucus and occasional liquid stool does not necessarily indicate a serious problem, especially if he is otherwise healthy. Keeping an eye on his overall health and maintaining a balanced diet with adequate hydration will be key in supporting his recovery. If you have any further concerns, don't hesitate to reach out to a healthcare professional for personalized advice.
