There is occasionally an irregular pulsating sensation in the left ear?

Hello Doctor, over the past month, I have been experiencing an irregular pulsating sensation in my left ear.
It feels like there is a "throbbing" or "pulsation" inside my ear, but it is not rhythmic like a heartbeat.
It is not tinnitus; it is just a sensation of pulsation that occurs irregularly.
This sensation suddenly appears when I am focused on something, such as looking at the computer or reading, but it does not last all night or all day; it happens intermittently during those activities (I do not feel it when watching TV).
I recall having this sensation occasionally in the past, but it was never frequent enough to pay attention to or to identify a pattern regarding when it occurs.
Recently, I noticed that it tends to happen when I come home and check my email or search for information.
My hearing is normal (I specifically tested my hearing in both ears before asking this question, and I can hear well in both) and I have not experienced any balance issues.
I did a quick online search using keywords related to my description and found that others have similar experiences, but I have not seen any professional medical opinions.
I initially speculated that it might be related to sleep, so I ensured I got about 6 to 7 hours of sleep last night, but it happened again today.

1.
What could be causing this sensation?
2.
Does the pulsating feeling in my left ear indicate any underlying issues?
P.S.
My blood pressure was normal when measured last month.
I kindly request your valuable professional medical explanation.
Thank you!

Hello: Based on your description, it is suspected that it may be related to the contraction of the small muscles in the middle ear, as it does not resemble pulsatile tinnitus caused by vascular factors, nor are there issues related to hearing or dizziness in the inner ear.
It is advisable to continue monitoring the situation and maintain a regular routine.
If there is no improvement, it is recommended to visit a hospital for relevant examinations.
Wishing you peace and health.

The sensation of irregular pulsations or a "pulsing" feeling in the left ear can be concerning, especially when it occurs frequently and disrupts your concentration. This phenomenon, often described as a rhythmic or pulsing sensation that does not align with your heartbeat, can be attributed to several potential causes.
1. Eustachian Tube Dysfunction: One of the most common reasons for such sensations in the ear is Eustachian tube dysfunction. The Eustachian tube connects the middle ear to the back of the nose and helps equalize pressure. If this tube is not functioning properly, it can lead to feelings of fullness, pressure, or even pulsation in the ear. This dysfunction can be exacerbated by allergies, sinus infections, or even changes in altitude.

2. Muscle Spasms: Another possibility is muscle spasms in the muscles surrounding the ear or within the Eustachian tube itself. These spasms can create a sensation of pulsing or fluttering. Stress and fatigue can contribute to muscle tension, which may explain why you notice this sensation more when you are focused on tasks like reading or working on the computer.

3. Tinnitus: While you mentioned that this does not feel like tinnitus, it is worth noting that some forms of tinnitus can present as a rhythmic sound or sensation. Tinnitus can be caused by a variety of factors, including exposure to loud noises, earwax buildup, or underlying health conditions.

4. Blood Flow Changes: Changes in blood flow or vascular issues can also lead to pulsatile tinnitus, where the sound of blood flow is perceived in the ear. This is often more noticeable when lying down or in quiet environments. However, since you mentioned that your blood pressure is normal, this may be less likely.

5. Neurological Factors: In some cases, irregular pulsations in the ear can be related to neurological conditions, although this is less common. If there are accompanying symptoms such as dizziness, balance issues, or changes in hearing, it would be prudent to seek further evaluation.

Given that your hearing is normal and you do not experience balance issues, the sensation you are experiencing may not indicate a serious underlying condition. However, it is essential to monitor the frequency and intensity of these sensations. If they become more frequent, are accompanied by pain, or if you notice any changes in your hearing or balance, it would be advisable to consult an ear, nose, and throat (ENT) specialist for a thorough examination.

In the meantime, consider the following recommendations:
- Manage Allergies: If you have a history of allergies, managing them effectively may help alleviate some of the symptoms.

- Stress Reduction: Engage in stress-reducing activities such as yoga, meditation, or deep-breathing exercises, which may help reduce muscle tension.

- Avoid Loud Noises: Protect your ears from loud environments, as this can exacerbate any underlying issues.

In conclusion, while the sensation of irregular pulsations in your left ear can be disconcerting, it is often manageable and may not indicate a severe problem. However, staying vigilant and seeking medical advice if symptoms persist or worsen is always a wise approach.
