Compression Discomfort and Phimosis Issues in Urology - Urology

Postural compression and issues related to the foreskin?

I would like to ask Dr.
Hsu about a recent observation: when sitting or lying down for a long time while reading or using my phone, I feel a discomfort as if a certain area is being compressed when I stand up.
This discomfort disappears after a slight stretch.
Is this normal? Or is it because I am overweight (BMI 30), causing the fat in the groin area to create an inward pressure? Additionally, I have a phimosis issue where the glans does not fully expose when not erect, and the foreskin tends to gather at the tip.
Even when erect, only about a quarter of the glans is exposed, but I can pull the foreskin down completely without feeling constricted.
Should I consider a circumcision? If I do not undergo circumcision, will it cause any issues during intercourse?

Mr. Hsu, 20~29 year old female. Ask Date: 2014/03/31

Dr. Xu Weikai reply Urology

Mr.
Hsu: Hello.
1.
Prolonged pressure can lead to poor blood circulation, and in severe cases, it may cause localized numbness.
If a certain position results in such symptoms, it is best to get up and move around every 20 minutes.
For example, when riding a bicycle, it is generally not recommended to sit continuously for more than 20 minutes.
2.
Individuals with a high BMI are not advised to undergo circumcision.
The fat covering the base of the penis can create an appearance of a shorter length.
If too much tissue is removed during surgery, there may be insufficient skin after weight loss.
If there are any unclear points, feel free to ask again for further discussion and research.
Online responses should only be used as a reference; it is still necessary to seek medical attention at healthcare facilities when needed.
This message is from Dr.
Hsu Wei-Kai, Director of the Urology Department, Taitung Hospital, Ministry of Health and Welfare.

Understanding compression discomfort and phimosis issues in urology is essential for addressing the concerns you have raised. Based on your description, it appears that you are experiencing discomfort in the groin area when sitting or lying down for extended periods. This discomfort may be exacerbated by your body weight, as a higher BMI can lead to increased pressure in the groin area, particularly if there is excess fat in the inguinal region.
Compression discomfort can occur when certain positions put pressure on the groin, leading to a sensation of tightness or discomfort. This is generally not a cause for concern if the discomfort resolves quickly upon changing positions. However, if the discomfort persists or is accompanied by other symptoms such as pain, swelling, or changes in urination, it would be advisable to consult a healthcare professional for further evaluation.

Regarding your phimosis, which is the condition where the foreskin cannot be fully retracted over the glans (head) of the penis, it is important to consider both the physical and functional implications. In your case, you mentioned that the glans is not exposed when flaccid and only partially exposed when erect. This can be a common presentation of phimosis, and while it may not cause immediate issues, it can lead to complications over time.

Phimosis can potentially interfere with sexual activity. In some cases, it may cause discomfort during intercourse, as the tight foreskin can create friction or pain. Additionally, if the foreskin is too tight, it may lead to tearing or other injuries during sexual activity. If you are experiencing discomfort or if the phimosis is causing you anxiety about sexual performance, circumcision (the surgical removal of the foreskin) may be recommended. This procedure can alleviate the tightness and prevent future complications.

However, circumcision is not the only option. Some individuals may benefit from conservative treatments, such as topical corticosteroids, which can help to loosen the foreskin and make retraction easier. Stretching exercises may also be recommended to gradually increase the elasticity of the foreskin. These options can be discussed with a urologist, who can provide guidance based on your specific situation.

In summary, the discomfort you are experiencing may be related to your body weight and positioning, but it is important to monitor the symptoms. Regarding your phimosis, if it is causing discomfort or concerns about sexual function, discussing circumcision or other treatment options with a urologist would be beneficial. They can provide a thorough evaluation and help you make an informed decision based on your needs and lifestyle. Remember, it is always best to seek professional medical advice tailored to your individual circumstances.
