Left lower abdominal tenderness?

I have pain in my lower left abdomen near the sigmoid colon, close to the hip bone, when I press down hard.
Normally, it doesn't hurt, but there is a dull sensation.
There are no lumps when I touch it; it feels flat, but pressing down causes pain.
Is there a problem with this?

More Info

Left lower abdominal pain can be a complex issue with various potential causes, especially when it is localized near the sigmoid colon and the lumbar region. Given your description of the pain being present only when pressure is applied and accompanied by a dull sensation, it is essential to consider several factors that could contribute to your symptoms.

Possible Causes
1. Muscle Strain or Ligamentous Injury: If you have engaged in physical activities that involve twisting or heavy lifting, it is possible that you have strained the muscles or ligaments in that area. This can lead to localized pain that is exacerbated by pressure.

2. Gastrointestinal Issues: Conditions such as diverticulitis, which is inflammation of the diverticula in the colon, can cause pain in the left lower abdomen. Although you mentioned that there are no palpable masses, it is still worth considering gastrointestinal causes, especially if you experience changes in bowel habits or other gastrointestinal symptoms.

3. Referred Pain: Sometimes, pain in the abdominal area can be referred from other structures, such as the kidneys or the spine. Given your history of back issues, it is possible that nerve irritation or referred pain from the lumbar region could manifest as abdominal discomfort.

4. Hernia: Although you mentioned that there are no visible lumps, certain types of hernias can present with pain without a noticeable bulge, especially if they are small or located deeper within the abdominal wall.

5. Psychosomatic Factors: Anxiety and stress can manifest physically, leading to muscle tension and discomfort in various parts of the body, including the abdomen. If you are prone to anxiety, this could be a contributing factor to your symptoms.

When to Seek Medical Attention
While occasional discomfort may not be a cause for alarm, persistent or worsening symptoms warrant further evaluation. Here are some signs that you should seek medical attention:
- Increased Pain: If the pain becomes more severe or is accompanied by other symptoms such as fever, nausea, vomiting, or changes in bowel habits, it is essential to consult a healthcare provider.

- Duration: If the pain persists for more than a few weeks without improvement, further investigation may be necessary to rule out underlying conditions.

- Functional Impact: If the pain interferes with your daily activities or quality of life, it is advisable to seek medical advice.

Recommended Next Steps
1. Consult a Healthcare Provider: A thorough evaluation by a healthcare professional, such as a gastroenterologist or an orthopedic specialist, may be necessary. They may recommend imaging studies, such as an ultrasound or CT scan, to assess the abdominal organs and structures.

2. Physical Examination: A physical examination can help determine if there is tenderness, swelling, or other signs that may indicate a specific condition.

3. Consider Physical Therapy: If muscle strain or tension is suspected, physical therapy may help alleviate pain and improve function through targeted exercises and stretches.

4. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their intensity, and any associated activities. This information can be valuable for your healthcare provider in diagnosing the issue.

In conclusion, while localized pain in the left lower abdomen can arise from various causes, it is crucial to monitor your symptoms and seek medical advice if they persist or worsen. A comprehensive evaluation will help identify the underlying cause and guide appropriate treatment.
