Ear problems?
Hello, doctor.
For many years, I've had a problem with my right ear where using cotton swabs results in brownish-yellow discharge.
I visited a clinic, and they said it could be due to ongoing inflammation.
This month, I've experienced the same issue with my left ear, which has discharge, pain, and decreased hearing clarity, almost like hearing loss.
Right now, my ears feel very uncomfortable, but I don't have a cold or anything else.
I am currently taking other medications, and I'm not sure if they are related because my ear condition seems to have worsened.
The clinic advised me to see an audiologist at a larger hospital.
Doctor, could my habit of cleaning my ears be causing this? Sometimes, even when I don't clean them, there is still discharge.
In the past, I had my right eardrum perforated several times.
Could this affect my hearing? Based on what I've described, what do you think is the underlying cause? Is it otitis media, otitis externa, or something else?

Dr. Zheng Jueyi reply Otolaryngology
Hi, Zhenzi: Hello, it could possibly be otitis externa.
I recommend medication along with ear drops for treatment.
Additionally, avoid digging in your ears and getting bath water into the ear canal.
If there is no improvement, you may need to go to the hospital for a bacterial culture to determine the appropriate medication for treatment.
Wishing you safety and health.
Sincerely, Dr.
Zheng Jueyi, Director of the ENT Department, Taoyuan Hospital, Ministry of Health and Welfare.

More Info
Hello! It sounds like you are experiencing a complex set of ear-related issues that have been ongoing for quite some time. Based on your description, there are several potential causes for your symptoms, including external ear canal inflammation (external otitis), middle ear infections (otitis media), and possibly issues related to your eustachian tube function.
Symptoms and Possible Causes
1. Ear Discharge: The presence of brownish-yellow discharge from your ears can indicate an infection or inflammation. This could be due to external otitis, often caused by water exposure or trauma from using cotton swabs, which can irritate the ear canal and lead to infection. Alternatively, if the discharge is coming from the middle ear, it could suggest a middle ear infection, especially if accompanied by pain and hearing loss.
2. Hearing Loss: You mentioned that your hearing isn't as clear as it used to be, which could be attributed to fluid accumulation in the middle ear (common in otitis media) or blockage in the ear canal due to wax buildup or inflammation. Repeated ear drum perforations can also lead to chronic hearing issues, as they can affect the normal functioning of the ear.
3. Pain and Discomfort: Pain in the ear, especially if it is new or worsening, should not be ignored. It can be a sign of an active infection or inflammation. If you have a history of ear drum perforations, this could also contribute to recurrent infections and discomfort.
4. Eustachian Tube Dysfunction: If you are experiencing a sensation of fullness or pressure in your ears, it may be related to eustachian tube dysfunction. This can occur due to allergies, sinus infections, or even changes in altitude, and can lead to fluid buildup in the middle ear.
Treatment Options
Given your symptoms, here are some recommendations:
1. Avoid Cotton Swabs: While it may be tempting to use cotton swabs to clean your ears, this can push wax deeper into the ear canal and cause irritation or injury. Instead, consider using ear drops designed to soften earwax or consult a healthcare professional for safe ear cleaning.
2. See an Ear, Nose, and Throat (ENT) Specialist: Since your symptoms have persisted and worsened, it would be wise to see an ENT specialist. They can perform a thorough examination, including a hearing test, and may use an otoscope to look inside your ears for signs of infection or other issues.
3. Medication: Depending on the diagnosis, your doctor may prescribe antibiotics if there is an infection or recommend anti-inflammatory medications to reduce pain and swelling. If allergies are contributing to your symptoms, antihistamines may also be helpful.
4. Hearing Evaluation: A hearing test can help determine the extent of any hearing loss and guide further treatment. If there is significant hearing loss, your doctor may discuss options such as hearing aids or other assistive devices.
5. Follow-Up Care: Regular follow-ups with your healthcare provider are essential, especially if you have a history of ear problems. This can help monitor your condition and prevent complications.
Conclusion
In summary, your symptoms could be related to a variety of ear conditions, including external otitis, otitis media, or eustachian tube dysfunction. It is crucial to seek further evaluation from an ENT specialist to determine the exact cause and appropriate treatment. Taking care of your ear health is vital, especially considering your history of ear drum perforations. Avoid self-treatment and consult with a healthcare professional for the best outcomes. Wishing you a speedy recovery!
