Itching of the skin on the hands and feet?

Hello, I have been experiencing unexplained itching in my hands and feet every night for the past two weeks, especially in my legs.
However, there are no visible skin abnormalities or swelling, but I have scratched my legs to the point where there are circular bruises.
This is quite bothersome.
P.S.
I have washed my sheets and comforter, and I do not have any pets at home, nor do I usually come into contact with animals.

Dr. Wu Yuxin reply Dermatology

There are no visible abnormalities or redness on the skin, which indeed makes diagnosis difficult.
Based on your description, I cannot provide a definitive diagnosis.
I recommend seeking medical attention promptly for a physical examination of the skin and a thorough medical history assessment to make an informed judgment.

Unexplained itching in the hands and feet, especially when it occurs at night, can be quite distressing. Given that you have experienced this for two weeks without any visible skin changes, such as redness or swelling, it is essential to consider various potential causes and solutions.

Possible Causes of Itching
1. Dry Skin: One of the most common causes of itching is dry skin, which can be exacerbated by environmental factors such as low humidity, especially in places like Arizona. This can lead to itching, particularly at night when the skin may lose moisture.

2. Allergic Reactions: Even if there are no visible signs, you might be experiencing a mild allergic reaction to something in your environment. This could include laundry detergents, fabric softeners, or even the materials in your bedding.
3. Nerve Issues: Conditions affecting the nerves, such as peripheral neuropathy, can lead to sensations of itching or tingling. This is often associated with diabetes, vitamin deficiencies, or other systemic issues.

4. Systemic Conditions: Certain systemic conditions, such as liver disease, kidney disease, or thyroid disorders, can cause generalized itching. These conditions may not always present with obvious symptoms initially.

5. Psychogenic Itching: Stress and anxiety can manifest physically, leading to sensations of itching without a clear dermatological cause.
6. Insect Bites or Infestations: Although you mentioned that there are no visible signs, it’s worth considering that you might have been bitten by insects or have an infestation that is not immediately apparent.

Solutions and Recommendations
1. Moisturize: Start by applying a good quality moisturizer to your hands and feet, especially before bedtime. Look for products that contain ingredients like ceramides or hyaluronic acid, which help retain moisture.

2. Avoid Irritants: Switch to hypoallergenic laundry detergents and avoid fabric softeners. Ensure that your bedding is made from breathable materials like cotton.

3. Cool Compresses: Applying cool compresses to the affected areas can provide immediate relief from itching.
4. Over-the-Counter Remedies: Consider using over-the-counter antihistamines, such as diphenhydramine (Benadryl), which can help alleviate itching, especially if it is related to an allergic reaction.

5. Hydration: Ensure you are well-hydrated, as dehydration can exacerbate dry skin and itching.

6. Consult a Healthcare Provider: Since the itching has persisted for two weeks and has led to bruising from scratching, it would be wise to consult a healthcare provider. They may recommend blood tests to check for underlying conditions such as liver function tests, kidney function tests, or thyroid function tests.
7. Dermatological Evaluation: If the itching continues, a dermatologist can assess your skin for any hidden conditions or recommend treatments such as topical corticosteroids or other medications.

8. Lifestyle Modifications: If stress is a contributing factor, consider stress-reduction techniques such as yoga, meditation, or regular exercise.

Conclusion
Unexplained itching can stem from various causes, ranging from dry skin to systemic health issues. It is crucial to monitor your symptoms and seek medical advice if they persist or worsen. Keeping a diary of your symptoms, including any potential triggers, can also be helpful for your healthcare provider in diagnosing the issue. Remember, while home remedies can provide relief, professional evaluation is essential for persistent or severe symptoms.
