Menstrual period
Hello Dr.
Zhong: I have a question regarding my menstrual cycle.
Before last year, my periods were relatively regular, although they would occasionally be delayed, but generally, they would come every month.
However, since the beginning of this year, they have become very irregular.
Up until now this year, I have only had 2 to 3 periods.
I am unsure if I need to see a specialist for further examination or if I should just take menstrual regulation pills to help.

Dr. Li Guanghan reply Obstetrics and Gynecology
Both lifestyle stress and ovarian disorders can lead to menstrual irregularities.
Therefore, it is not sufficient to only take menstrual regulation pills; a visit to a gynecologist for examination is necessary before regulating the menstrual cycle.

Irregular menstruation, or oligomenorrhea, can be a common concern for many women and may arise from various underlying causes. It is essential to understand when to seek medical advice regarding this issue. Based on your description, it appears that your menstrual cycle has significantly changed over the past year, with only 2 to 3 occurrences this year. This level of irregularity warrants a closer examination by a healthcare professional.
There are several potential causes of irregular menstruation, including hormonal imbalances, stress, significant weight changes, excessive exercise, polycystic ovary syndrome (PCOS), thyroid disorders, and other medical conditions. Hormonal fluctuations, particularly involving estrogen and progesterone, can lead to changes in the menstrual cycle. For instance, conditions like PCOS can cause irregular ovulation, leading to infrequent or absent periods. Thyroid disorders, both hypothyroidism and hyperthyroidism, can also disrupt menstrual regularity.
Given that your menstrual cycle was previously regular, the sudden change could indicate an underlying issue that may require further investigation. It is advisable to consult with a healthcare provider who can perform a thorough evaluation, which may include a physical examination, blood tests to check hormone levels, and possibly imaging studies like an ultrasound to assess the ovaries and uterus.
While over-the-counter medications or herbal supplements may seem appealing for regulating your cycle, it is crucial to approach this with caution. Self-medicating without understanding the underlying cause of your irregularity can mask symptoms and potentially lead to further complications. For example, if the irregularity is due to a hormonal imbalance, simply taking menstrual regulation pills may not address the root cause and could lead to more significant health issues down the line.
In summary, given the significant change in your menstrual cycle, it is advisable to seek medical advice rather than relying solely on over-the-counter treatments. A healthcare provider can help identify any underlying conditions and recommend appropriate treatment options tailored to your specific needs. Early intervention can be beneficial in managing any potential health issues and restoring your menstrual cycle to a more regular pattern.
In conclusion, if you are experiencing irregular menstruation, especially if it is accompanied by other symptoms such as weight changes, excessive hair growth, acne, or significant pain, it is essential to consult with a healthcare professional. They can provide a comprehensive assessment and guide you toward the most effective treatment options, ensuring that your reproductive health is well-managed.
