Missed a birth control pill?

Hello, doctor.
My last menstrual period started on July 13 and lasted about five days.
On the fifth day, I began taking the oral contraceptive pill (Yaz low-dose).
I went on a trip on August 1 and 2 (during which I had sexual intercourse) and forgot to bring my pills.
I took a pill on the morning of August 3 and another one at noon.
However, I just noticed some light bleeding in the bathroom (very little and a pale color).
I'm not sure if this is my menstrual cycle.
Should I continue taking the contraceptive pills?

Bi Bi, 30~39 year old female. Ask Date: 2014/08/03

Dr. Lü Lizheng reply Obstetrics and Gynecology

Please continue with the treatment as suggested.
If there is significant bleeding, discontinue the medication.
Sincerely, Dr.

When you forget to take your birth control pills, it can understandably cause concern, especially if you've had unprotected intercourse during that time. In your case, since you started taking the low-dose oral contraceptive (like Yaz) on the fifth day of your menstrual cycle, it's important to understand how to proceed after missing doses.

First, let's clarify the situation: you began taking the pill after your period, and then you missed taking it during your trip. You mentioned that you took the missed pills as soon as you remembered, which is a good step. The general recommendation for missed pills is to take the missed dose as soon as you remember, and then continue taking the remaining pills at your regular schedule. If you miss two or more pills in a row, the guidance can vary slightly depending on the type of pill you are using.

Since you experienced some light bleeding, it could be a breakthrough bleed, which is not uncommon when you miss pills. This bleeding can occur due to hormonal fluctuations caused by the missed doses. It’s important to note that this does not necessarily mean you are having your period, especially since you just started the pill.

Here’s what you should do next:
1. Continue Taking Your Pills: Since you have already taken the missed pills, continue taking the rest of your pills as scheduled. This will help maintain the hormone levels in your body and reduce the risk of unintended pregnancy.

2. Use Backup Contraception: Since you missed pills and had unprotected intercourse, it’s advisable to use a backup method of contraception, such as condoms, for at least the next seven days. This is especially important if you missed pills in the first week of your cycle, as this is when you are most vulnerable to pregnancy.

3. Monitor Your Symptoms: Keep an eye on any further bleeding or changes in your cycle. If the bleeding continues or if you have any other concerning symptoms, consider reaching out to your healthcare provider for advice.

4. Consider Emergency Contraception: If you are worried about the possibility of pregnancy due to unprotected intercourse and missed pills, you might want to consider emergency contraception (like Plan B) if it’s within the appropriate time frame (typically within 72 hours after unprotected sex). However, it’s best to consult with your healthcare provider regarding this option.

5. Consult Your Healthcare Provider: If you have any doubts or if the bleeding persists, it’s a good idea to consult your healthcare provider. They can provide personalized advice based on your medical history and current situation.

In summary, it’s crucial to continue taking your birth control pills as directed, use backup contraception, and monitor your symptoms. If you have any concerns about your health or the effectiveness of your birth control, do not hesitate to reach out to a healthcare professional for guidance. Remember, managing birth control effectively requires diligence, and it’s always better to ask for help when you’re unsure.
