Blood in the stool?

Hello Doctor, I previously had a hard bump at the anal opening that caused slight pain when squeezed.
I thought it might be hemorrhoids, but I didn't pay much attention to it, and it went away after a while.
It occurred again later, but it also resolved after some time.
Currently, I am a student with a regular schedule and have daily bowel movements.
Recently, I started taking a green dietary fiber supplement for almost two months, which has helped with smoother bowel movements.
However, today my stool has become harder and thicker.
During my first bowel movement, I noticed a fresh red blood stain about the size of a coin on the toilet paper, but I forgot to observe the stool.
During the second bowel movement, there was no blood on the toilet paper, but I saw some blood at one end of the stool.
Although it wasn't much, it was still noticeable and bright red.
I haven't had a health check-up in a long time.
Should I go to the hospital for an examination? I'm quite worried because my mother has a history of cancer, although it wasn't colon cancer.
Please advise me, thank you.

Dr. Ke Fangxu reply Surgery

If there is slight bleeding after passing hard stools, it is likely caused by an anal fissure.
It is recommended to increase the intake of dietary fiber and to take warm sitz baths.
Over-the-counter medications that claim to aid in bowel movements should be used sparingly.
If there are concerns or persistent bowel issues, it is advisable to consult a healthcare professional for evaluation and possible prescription medication.
If necessary, a colonoscopy can also be arranged.

When it comes to rectal issues, particularly the presence of blood in the stool, it is essential to take the situation seriously and seek medical attention when necessary. Based on your description, there are several factors to consider regarding your symptoms and the appropriate course of action.

Firstly, the presence of bright red blood on toilet paper or mixed with stool can indicate several conditions, ranging from benign to more serious issues. Common causes of bright red blood in the stool include hemorrhoids, anal fissures, diverticular disease, and, in some cases, colorectal polyps or cancer. Given your previous experience with hard lumps near the anus, which you suspected might be hemorrhoids, it is possible that your current symptoms could be related to that condition. Hemorrhoids can cause bleeding, especially if there is straining during bowel movements or if the stool is hard and difficult to pass.

However, the fact that you have a family history of cancer, even if it is not directly related to colorectal cancer, adds an additional layer of concern. Family history can increase the risk of various types of cancers, including colorectal cancer. Therefore, it is prudent to be cautious and proactive about your health.

In your case, since you have noticed blood in your stool and have a family history of cancer, it is advisable to seek medical attention. A healthcare provider can perform a thorough evaluation, which may include a physical examination, a review of your medical history, and possibly diagnostic tests such as a colonoscopy. A colonoscopy is particularly useful for visualizing the inside of the colon and rectum and can help identify any abnormalities, such as polyps or tumors, that may require further investigation or treatment.

Additionally, you mentioned that you have been taking a fiber supplement to aid in bowel movements. While fiber is generally beneficial for digestive health, it is essential to ensure that you are also drinking enough water and maintaining a balanced diet to prevent constipation and hard stools. If your stool has become harder, it may be worth reviewing your diet and hydration levels to see if adjustments can be made to improve your bowel health.

In summary, while your symptoms may be related to a benign condition such as hemorrhoids, the presence of blood in the stool, especially in the context of a family history of cancer, warrants a medical evaluation. Do not hesitate to reach out to a healthcare professional to discuss your symptoms and concerns. Early detection and intervention are key in managing any potential health issues effectively. Remember, it is always better to err on the side of caution when it comes to your health.
