Abnormal bowel movements?

Hello, doctor.
About two and a half years ago, I underwent a colonoscopy, during which a small hyperplastic polyp was found in the sigmoid colon.
It was removed and tested, and the results were normal.
Six months ago, I had a follow-up colonoscopy that reached the junction of the ascending and transverse colon (approximately 60 cm), and no polyps were found.
Recently, I have been experiencing occasional diarrhea within the past month, but it resolves quickly, and the next day my bowel movements return to normal (sometimes formed, sometimes softer).
I would like to ask if this is considered abnormal? Could it be related to a malignant tumor? Should I undergo another colonoscopy, and if so, how often?

Guang, 30~39 year old female. Ask Date: 2014/09/30

Hello, sir: 1.
It is recommended to first check for fecal occult blood and to adjust medications and diet.
2.
If discomfort persists, please discuss with your physician whether further examination is necessary.

When it comes to changes in bowel movements, it's essential to understand that variations can occur for numerous reasons, and not all changes indicate a serious underlying condition. Based on your description, it seems that you have experienced some fluctuations in your bowel habits over the past month, including occasional diarrhea followed by normal stools. Given your medical history of a benign polyp removal and recent colonoscopy, it's understandable to be concerned about the implications of these changes.

Firstly, it's important to note that bowel habits can be influenced by various factors, including diet, stress, hydration, and physical activity. For instance, dietary changes, such as increased fiber intake or the consumption of certain foods, can lead to variations in stool consistency. Stress and anxiety can also significantly impact gut health, potentially leading to symptoms like diarrhea or constipation.
In your case, the intermittent diarrhea followed by normal stools could be attributed to several benign causes, such as dietary indiscretion, mild infections, or even stress-related gastrointestinal disturbances. The fact that you have a history of a benign polyp and have undergone recent surveillance colonoscopy without any new findings is reassuring. Generally, significant changes in bowel habits, especially if accompanied by alarming symptoms such as blood in the stool, persistent abdominal pain, or unexplained weight loss, warrant further investigation.

Regarding your concern about the potential link to malignancy, it is crucial to understand that while changes in bowel habits can be a symptom of colorectal cancer, they are typically accompanied by other significant symptoms. The absence of alarming symptoms in your case, along with the recent normal colonoscopy results, suggests that the likelihood of a malignant process is low.
As for the frequency of follow-up colonoscopies, guidelines generally recommend that individuals with a history of polyps undergo surveillance colonoscopy every three to five years, depending on the type and number of polyps found. Since you had a benign polyp removed and your recent colonoscopy was clear, you may not need to undergo another colonoscopy for some time unless you experience new concerning symptoms or changes.

In summary, while changes in bowel habits can be concerning, your recent colonoscopy results and the nature of your symptoms suggest that there may not be an immediate cause for alarm. However, if your symptoms persist or worsen, or if you develop any new symptoms, it would be prudent to consult your healthcare provider for further evaluation. Maintaining a balanced diet, managing stress, and staying hydrated can also contribute positively to your digestive health. If you have any ongoing concerns, don't hesitate to reach out to your healthcare provider for personalized advice and potential further investigation.
