My two sides are not quite the same?
I am 21 years old and had surgery for left-sided varicocele when I was 12.
I believe I have recovered, but I still feel that my left testicle is larger, and upon examination, it indeed seems so.
I have read some literature stating that the left testicle is usually larger, while others claim they should be the same size.
What is the correct information? Last year, I also noticed that my left epididymis is larger and harder than the right one, and it can be slightly painful when I apply pressure.
I consulted a doctor who diagnosed it as inflammation and prescribed anti-inflammatory medication, but my condition did not improve.
I have seen the doctor twice, but it seems there are no significant long-term effects, so I haven't paid much attention to it.
However, recently, my libido has suddenly decreased, and I am starting to worry if there is an underlying issue.
Could you please help me with this? Thank you!

Varicocele generally has a higher incidence on the left side.
As for the size of the testicles, there may be some variation, but as long as they are not too small, they are considered normal.
If there is swelling of the epididymis, it is important to consider whether it is due to inflammation.
Epididymitis typically affects sperm count but does not have a direct relationship with libido.
The treatment for epididymitis usually involves a course of antibiotics lasting four weeks.

Understanding testicular size differences and related health concerns can be quite complex, especially considering your medical history of left-sided varicocele and the symptoms you are currently experiencing.
Firstly, it's important to note that anatomical variations in testicular size are common. In many males, the left testicle is often slightly larger than the right, and this can be attributed to various factors, including the position of the testicles in the scrotum and individual anatomical differences. However, significant discrepancies in size or any changes in consistency or pain should not be overlooked.
Your history of varicocele, which is an enlargement of the veins within the scrotum, can lead to changes in testicular size and function. After surgical treatment, it is not uncommon for some men to experience lingering effects, including size differences or discomfort. The inflammation you mentioned in the epididymis (the tube that carries sperm) could also contribute to the sensation of increased size and hardness. Epididymitis, which is often caused by infections or other inflammatory processes, can lead to pain and swelling, and if left untreated, it can potentially affect fertility and hormone levels.
The fact that you are experiencing a decrease in libido is concerning and warrants further investigation. Hormonal imbalances, particularly involving testosterone, can be influenced by various factors, including testicular health. The testicles are responsible for testosterone production, and any issues affecting their function can lead to changes in sexual desire.
Given your symptoms of persistent pain, size differences, and changes in libido, it is crucial to follow up with a healthcare provider, preferably a urologist, who specializes in male reproductive health. They may recommend further diagnostic tests, such as an ultrasound, to assess the structure of your testicles and epididymis, check for any abnormalities, and rule out conditions such as tumors or cysts. Blood tests to measure hormone levels, including testosterone, can also provide valuable information regarding your sexual health.
In summary, while some variation in testicular size is normal, the combination of your symptoms—persistent pain, noticeable size differences, and decreased libido—suggests that further evaluation is necessary. It is essential to address these concerns with a healthcare professional who can provide a thorough examination and appropriate treatment options. Early intervention can help prevent potential complications and ensure your reproductive health is maintained.
