Ligament surgery
I just had surgery a month ago for an artificial anterior cruciate ligament (ACL) and a repair of the medial meniscus.
Due to a history of gout, I experienced an acute gout attack three days post-surgery.
My ankle has been swollen and has limited range of motion.
I also tend to have muscle cramps, which is affecting my rehabilitation progress.
The wound is currently scabbed over.
I would like to know, aside from seafood, what other foods should I avoid? Can I consume sesame oil or ginger? Is it safe to eat foods cooked with Chinese medicinal herbs?

Dr. Chen Yongxue reply Orthopedics
For Western medicine, there are no dietary restrictions required after cruciate ligament and meniscus repair.
Thank you.

More Info
After undergoing surgery for an ACL (anterior cruciate ligament) reconstruction and ligament repair, it is crucial to pay attention to your dietary choices to support recovery and manage any pre-existing conditions, such as gout. Here are some dietary guidelines and considerations to help you navigate your post-surgery recovery effectively.
General Dietary Guidelines Post-Surgery
1. Hydration: Staying well-hydrated is essential for recovery. Aim to drink plenty of water throughout the day. Proper hydration can help reduce swelling and support overall healing.
2. Balanced Diet: Focus on a balanced diet that includes a variety of food groups:
- Proteins: Essential for tissue repair. Include lean meats, poultry, fish (avoid shellfish due to gout), eggs, and plant-based proteins like beans and lentils.
- Fruits and Vegetables: Rich in vitamins, minerals, and antioxidants that support healing. Aim for a colorful variety to maximize nutrient intake.
- Whole Grains: Foods like brown rice, quinoa, and whole-grain bread provide necessary carbohydrates for energy and fiber for digestive health.
3. Healthy Fats: Include sources of healthy fats, such as avocados, nuts, seeds, and olive oil. These can help reduce inflammation and support overall health.
Specific Considerations for Gout Management
Since you have a history of gout, it is vital to avoid foods that can trigger flare-ups. Here are some specific dietary restrictions:
1. Avoid Purine-Rich Foods: Foods high in purines can exacerbate gout symptoms. These include:
- Certain Seafood: Shellfish, sardines, and anchovies should be avoided.
- Red Meat: Limit consumption of beef, lamb, and pork.
- Organ Meats: Liver and other organ meats are particularly high in purines.
- Certain Fish: While some fish are acceptable, avoid those known to be high in purines.
2. Limit Alcohol: Alcohol, especially beer and spirits, can increase uric acid levels and should be limited or avoided.
3. Watch Out for Sugary Foods: High-fructose corn syrup and sugary beverages can also contribute to increased uric acid levels.
Specific Foods and Ingredients
Regarding your questions about specific foods:
- Sesame Oil (麻油): Generally, sesame oil is considered a healthy fat and can be used in moderation. However, if you notice any adverse reactions or increased swelling, it may be best to limit its use.
- Ginger (生薑): Ginger has anti-inflammatory properties and can be beneficial in small amounts. It may help with pain relief and should be safe to consume unless you have a specific allergy or sensitivity.
- Chinese Herbal Ingredients: The safety of consuming foods prepared with Chinese herbs depends on the specific herbs used. Some herbs may have anti-inflammatory properties, while others could potentially affect uric acid levels. It is advisable to consult with a healthcare provider or a registered dietitian familiar with your medical history before incorporating these into your diet.
Conclusion
In summary, after ACL and ligament repair surgery, focus on a balanced diet rich in proteins, fruits, vegetables, and healthy fats while avoiding purine-rich foods to manage gout. Stay hydrated and consult with a healthcare professional for personalized dietary advice, especially regarding the use of specific ingredients like sesame oil, ginger, and Chinese herbs. This approach will not only support your recovery but also help manage your gout effectively. Always listen to your body and adjust your diet based on how you feel during your recovery process.
