Regarding hematospermia?
Hello Doctor, since experiencing hematospermia last week, I have noticed that during bowel movements, if I exert myself, my urine has blood in it.
I'm not sure if this is related to the blood from the hematospermia.
However, when I urinate normally, everything seems fine; it's only during bowel movements when I strain that my urine has some dark red coloration.
Is it possible that old blood could be expelled this way? Thank you for your response!

Dr. Xu Weikai reply Urology
Hello, your hematospermia may be caused by a rupture in the urethra or urethritis.
Constipation can lead to straining, which increases blood pressure and may cause previously unhealed blood vessels to rupture again.
It is advisable to seek medical diagnosis and treatment.
If you have any further questions, feel free to ask, and we can discuss and research together.

Hematospermia, or the presence of blood in semen, can be a concerning symptom for many individuals. In your case, it seems that you are experiencing blood in your urine during bowel movements, particularly when straining. This situation raises several important considerations regarding the potential causes and implications of your symptoms.
Firstly, it is essential to understand that the presence of blood in urine (hematuria) can arise from various sources within the urinary tract, including the kidneys, ureters, bladder, and urethra. When you mention that blood appears only during bowel movements and not during urination, it suggests that the source of the bleeding may not be directly related to the urinary system but could be influenced by the pressure exerted during straining.
One possible explanation for your symptoms could be the presence of hemorrhoids or anal fissures. These conditions can cause bleeding during bowel movements, and if you are straining, it is possible that the increased abdominal pressure could lead to blood being present in your urine as well. This could occur if there is a connection between the rectal and urinary systems, although it is relatively uncommon.
Another consideration is that straining during bowel movements can lead to increased pressure in the pelvic region, which may exacerbate any existing conditions, such as urinary tract infections (UTIs) or prostate issues, particularly in males. If there is any inflammation or irritation in the urinary tract, it could lead to bleeding that is more noticeable when straining.
Regarding your question about whether old blood could be expelled during bowel movements, it is indeed possible. Blood that has been present in the urinary tract for some time may not be immediately noticeable until there is a change in pressure or activity, such as during a bowel movement. However, it is crucial to differentiate between blood that is fresh and blood that is older, as this can indicate different underlying issues.
Given your symptoms, it is advisable to consult a healthcare professional for a thorough evaluation. They may recommend a urinalysis to check for the presence of red blood cells, as well as imaging studies or a cystoscopy to visualize the urinary tract. Additionally, a rectal examination may be warranted to assess for hemorrhoids or other anal conditions.
In summary, while it is possible that the blood you are noticing during bowel movements is related to your previous episode of hematospermia, it is essential to investigate further to determine the exact cause. Conditions such as hemorrhoids, urinary tract infections, or prostate issues could all contribute to your symptoms. Seeking medical attention will provide you with a clearer understanding of your situation and appropriate management options.
