Will this cause any issues with the kidneys?

Dear Dr.
Chen,
I have recently felt that I am taking quite a few medications and would like to ask if this could put a burden on my kidneys.
My daily medication schedule is as follows:
1.
Upon waking up, I take 6 capsules of Jin Yong Shen Ming Mu Wan (traditional Chinese medicine).
2.
After breakfast, I take 1 tablet of propranolol 10 mg, 1 tablet of paran (tinten), 1 tablet of chlormezanone, and 1 tablet of fludiazepam 0.25 mg.
3.
Before lunch, I take another 6 capsules of Jin Yong Shen Ming Mu Wan, 1 lutein, and 1 fish oil.
4.
After my afternoon nap, I take 1 tablet of propranolol 10 mg, 1 tablet of paran (tinten), and 1 tablet of chlormezanone.
5.
Before dinner, I take another 6 capsules of Jin Yong Shen Ming Mu Wan.
6.
After dinner, I take 1 tablet of propranolol 10 mg, 1 tablet of paran (tinten), 1 tablet of chlormezanone, and 1 tablet of fludiazepam 0.25 mg.
7.
Before bed, I take 1 tablet of fludiazepam 0.25 mg.
I would like to ask the following questions:
1.
Is this regimen harmful to my kidneys?
2.
My blood pressure at home is approximately 134/87.
Should I continue taking my blood pressure medication?
3.
Should I inform my doctor that I am taking Jin Yong Shen Ming Mu Wan?
4.
I had a blood test done around October 2014 at a general laboratory.
How often is it generally recommended to have blood tests?
Thank you for your attention to these questions.
Wishing you peace and good health.
Sincerely,
Chen Xiao Yan

Dr. Chen Zonglin reply Family Medicine

1.
The impact of Western medicine on kidney function is minimal.
2.
After taking the medication, the blood pressure is 134/87; however, it may rise above 140/90 after discontinuation.
Currently, it is not recommended to stop the medication, and blood pressure should continue to be monitored.
3.
If the physician can understand the patient's complete medication history, they will be able to assess the condition more accurately.
4.
For general health screenings, it is recommended that individuals under 40 have a check-up every five years, those aged 40 to 65 every three years, and those over 65 annually.
If there are any abnormal findings, the follow-up interval should be determined based on those specific items.
Wishing you good health and happiness.

When it comes to managing medications and their potential impact on kidney health, it's essential to approach the situation with a comprehensive understanding of both the medications involved and the patient's overall health status. Based on your medication routine and concerns, let's address your questions systematically.

1. Impact on Kidney Health: The medications you are taking, including propranolol, chlormezanone, fludiazepam, and various supplements, can have varying effects on kidney function. Propranolol, a beta-blocker, is generally safe for the kidneys, but it is crucial to monitor blood pressure and kidney function regularly. Chlormezanone and fludiazepam are medications that can affect the central nervous system and may not directly impact kidney function, but their sedative effects should be considered, especially in older adults or those with existing health issues. The herbal supplement "金勇腎明目丸" (Jin Yong Shen Ming Mu Wan) is a traditional Chinese medicine that may have unknown effects on kidney health, particularly if it contains nephrotoxic ingredients or interacts with your other medications. Therefore, it is advisable to consult with a healthcare provider familiar with herbal supplements to assess any potential risks.

2. Blood Pressure Management: Your recorded blood pressure of 134/87 mmHg is slightly elevated, indicating that you may still benefit from antihypertensive therapy. It is essential to discuss this with your healthcare provider, as they can evaluate whether your current medication regimen is adequate or if adjustments are necessary. Maintaining blood pressure within a healthy range is crucial for kidney health, as uncontrolled hypertension can lead to kidney damage over time.

3. Informing Your Doctor: Yes, it is vital to inform your healthcare provider about all medications and supplements you are taking, including "金勇腎明目丸." This information helps your doctor assess potential drug interactions, side effects, and overall treatment efficacy. Transparency about your medication routine allows for better management of your health and can help prevent adverse effects.

4. Frequency of Blood Tests: The frequency of blood tests can vary based on individual health conditions, medications, and risk factors. Generally, for someone on multiple medications or with existing health concerns, it is advisable to have kidney function tests (such as serum creatinine and estimated glomerular filtration rate) at least once a year. However, if there are changes in your health status, medication adjustments, or if you have risk factors for kidney disease, more frequent monitoring may be warranted. Discuss with your healthcare provider to determine the best schedule for your situation.

In summary, while your current medication routine may not inherently harm your kidneys, it is crucial to monitor kidney function regularly, manage blood pressure effectively, and maintain open communication with your healthcare provider about all medications and supplements. Regular check-ups and blood tests will help ensure that any potential issues are identified early, allowing for timely interventions to protect your kidney health. Always consult with your healthcare provider before making any changes to your medication regimen or if you have concerns about your health.
