ABC: Hello, the tingling sensation may be due to an infection or trauma.
It is recommended to perform a urinalysis and bacterial culture.
Additionally, if there has been sexual contact, it is best for both partners to get tested for sexually transmitted infections (STIs).

Understanding pain during erection, particularly when it is localized near the urethra close to the testicles, can be concerning and warrants a thorough evaluation. The sensation of sharp or stabbing pain during an erection, especially when it occurs only during exertion or pressure, can be attributed to several potential causes.

1. Urethral Irritation or Injury: The urethra is a sensitive structure that can become irritated due to various factors, including infections (like urethritis), trauma, or even excessive friction during sexual activity. If there has been any recent activity that could have caused irritation, this might explain the pain.

2. Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, which can lead to painful erections and curvature of the penis. While this typically presents with more pronounced symptoms, it is worth considering if there is any history of trauma or unusual curvature.

3. Testicular Issues: Sometimes, pain in the area near the urethra can be referred pain from the testicles. Conditions such as epididymitis (inflammation of the epididymis) or testicular torsion can cause discomfort that may be felt during an erection.

4. Muscle Strain: The pelvic floor muscles play a significant role during an erection. If these muscles are strained or tense, it can lead to discomfort or pain during erections. This is especially true if there has been any recent physical activity that might have overexerted these muscles.

5. Nerve Compression or Irritation: Nerves that innervate the genital area can sometimes become compressed or irritated, leading to pain during specific activities, including erections. This could be due to posture, prolonged sitting, or other mechanical factors.

6. Psychological Factors: Anxiety or stress related to sexual performance can sometimes manifest as physical symptoms, including pain during erections. This is often more complex and may require addressing both the physical and psychological aspects.

Recommendations for Management
- Medical Evaluation: It is crucial to consult a healthcare provider, preferably a urologist, who can perform a physical examination and possibly recommend tests such as urinalysis, ultrasound, or other imaging studies to rule out underlying conditions.

- Avoid Irritants: If you suspect that certain activities or products (like soaps or lubricants) may be irritating the area, consider avoiding them until you can consult with a doctor.

- Pelvic Floor Exercises: If muscle strain is suspected, pelvic floor exercises (Kegel exercises) may help strengthen the muscles and alleviate discomfort.

- Pain Management: Over-the-counter pain relief, such as NSAIDs (ibuprofen or naproxen), may help manage discomfort, but should only be used as directed and not as a long-term solution without medical advice.

- Follow-Up: If the pain persists or worsens, it is essential to follow up with your healthcare provider to explore further diagnostic options and treatment plans.

In conclusion, while occasional discomfort during an erection can be benign, persistent or severe pain should not be ignored. A thorough evaluation by a healthcare professional is essential to determine the underlying cause and appropriate treatment.
