Follow-up considerations for hematospermia?
Hello Doctor, I experienced hematospermia in 2012, where my semen appeared completely pink.
After a week of treatment with anti-inflammatory medication, I recovered.
Recently, after having sexual intercourse a week ago, I noticed blood in my semen again.
After consulting another specialist, I was prescribed a month-long course of antibiotics and advised to have a semen analysis after completing the treatment.
I would like to ask if my condition is infection-related, should I completely avoid sexual intercourse? Would not using a condom put my partner at risk of infection or exacerbate my own condition?
Additionally, during the recent instances of hematospermia, I held back from ejaculation multiple times, and towards the end, my penis felt very swollen and uncomfortable.
After that, I ejaculated normally, but the next day I experienced hematospermia.
Could it be possible that excessive engorgement and subsequent rupture of blood vessels led to the occurrence of hematospermia?

Hello,
1.
Hematospermia does not necessarily indicate an infection.
An infection would typically cause an infection in the female partner.
However, if there is uncertainty, using a condom can help prevent transmission.
2.
If sexual activity is excessively vigorous or prolonged, it can indeed lead to swelling that may cause bleeding.
If there are any unclear points, feel free to ask again for further discussion and research.

Post-ejaculatory hematospermia, or the presence of blood in the semen, can be a concerning symptom for many men. It can arise from various causes, including infections, inflammation, trauma, or even benign conditions. Given your history of experiencing hematospermia in 2012 and the recent recurrence after sexual activity, it is crucial to consider several factors regarding your situation.
Firstly, if your current symptoms are suspected to be related to an infection, it is generally advisable to avoid sexual intercourse until the infection is fully treated. Engaging in sexual activity while you have an active infection can potentially exacerbate your condition and may also pose a risk of transmitting the infection to your partner, especially if the infection is sexually transmitted. Using condoms can reduce the risk of transmission, but it is still best to refrain from sexual activity until you have completed your antibiotic course and received medical clearance.
Regarding your experience of blood in the semen following a period of intense sexual arousal and restraint, it is possible that the excessive engorgement of blood vessels in the penis could lead to minor trauma or rupture of small blood vessels, resulting in hematospermia. This phenomenon can occur when there is significant pressure or strain on the vascular structures during sexual activity, especially if there is prolonged arousal without ejaculation. However, this is typically a benign cause and should resolve on its own, particularly if it is not accompanied by other concerning symptoms such as pain, swelling, or persistent bleeding.
It is essential to follow your healthcare provider's recommendations, including completing the prescribed course of antibiotics and undergoing any suggested semen analysis or follow-up tests. These evaluations can help determine the underlying cause of your hematospermia and guide appropriate treatment. If the symptoms persist or worsen, or if you experience additional symptoms such as fever, severe pain, or changes in urination, you should seek medical attention promptly.
In summary, while post-ejaculatory hematospermia can be alarming, it is often manageable with appropriate medical care. Avoiding sexual activity during treatment, adhering to prescribed medications, and monitoring your symptoms are key steps in addressing this condition. If you have any further concerns or questions, do not hesitate to reach out to your healthcare provider for personalized advice and support.
