Missed menstruation and breast tenderness?

Hello, doctor.
Since I was young, my menstrual periods have only come every few months, but I usually experience breast tenderness and increased discharge one to two weeks before my period.
However, this time my period has been delayed, and I only have a small amount of spotting and light yellow discharge.
The breast tenderness has persisted for nearly a month, and sometimes I feel a dull pain in my lower right abdomen.
Could this be menopause, or is it something else? Thank you for your guidance!

Yi Qingfang, 40~49 year old female. Ask Date: 2015/03/10

Dr. Lin Wenbin reply Obstetrics and Gynecology

You need to visit an outpatient clinic to check your hormone levels.
It may not necessarily be menopause, but it cannot be ruled out.
Other factors, such as thyroid function, can also affect menstruation.

Delayed menstruation and breast pain can be concerning symptoms, especially when they persist for an extended period. Based on your description, it seems that you have a history of irregular menstrual cycles, which can be influenced by various factors, including hormonal imbalances, lifestyle, and underlying health conditions.

Firstly, it is essential to understand that breast pain, also known as mastalgia, can occur due to hormonal fluctuations, particularly in relation to the menstrual cycle. Many women experience breast tenderness or swelling in the days leading up to their period due to changes in estrogen and progesterone levels. However, if you are experiencing persistent breast pain for nearly a month, it may indicate that there is something more than just the typical premenstrual symptoms at play.

The presence of a small amount of blood and yellowish discharge, along with breast pain, could suggest several possibilities. One common cause could be hormonal imbalances, which may lead to conditions such as polycystic ovary syndrome (PCOS) or other endocrine disorders. These conditions can disrupt the normal menstrual cycle and lead to symptoms like breast tenderness and unusual discharge.

Another possibility is that you may be experiencing symptoms related to perimenopause, the transitional phase before menopause, which can begin several years before the last menstrual period. During this time, hormonal fluctuations can cause irregular periods, breast pain, and other symptoms such as mood swings and hot flashes. However, given your age and the history of irregular cycles, it would be prudent to consult with a healthcare provider to rule out other potential causes.

Additionally, the right lower abdominal discomfort you mentioned could be related to ovarian issues, such as cysts or other gynecological conditions. It is essential to have a thorough evaluation, including a pelvic exam and possibly imaging studies like an ultrasound, to assess the health of your reproductive organs.

In terms of management, if hormonal imbalances are confirmed, your healthcare provider may recommend lifestyle changes, such as maintaining a healthy diet, regular exercise, and stress management techniques. In some cases, hormonal therapy may be considered to help regulate your menstrual cycle and alleviate breast pain.

It is crucial to seek professional medical advice to address your symptoms adequately. A gynecologist can provide a comprehensive evaluation and guide you through the necessary tests to determine the underlying cause of your symptoms. They can also discuss treatment options tailored to your specific needs.

In summary, while breast pain and delayed menstruation can be related to hormonal changes, persistent symptoms warrant further investigation. Consult with a healthcare provider to explore the potential causes and receive appropriate care. Remember, early intervention can lead to better management of your symptoms and overall health.
