Frequent urination issues?
Hello Doctor, in September 2013, during an ejaculation, I felt a sharp pain in my urethra, and then I started experiencing frequent urination, abdominal bloating, and the urge to urinate again within half an hour after finishing.
Sometimes I felt the need to urinate every 5-10 minutes, along with a burning sensation in the urethra and at the urethral opening.
I went to the hospital for a urine test, which came back normal.
After the doctor performed a prostate massage, I was tested again, and the results were still normal.
I was later diagnosed with non-bacterial prostatitis.
I took medication for a while, but the feeling of frequent urination persisted.
In 2014, I visited the urology department almost every month, but all test results were normal, including prostate fluid analysis.
I continued taking medication for over a year, but my symptoms did not improve, and I felt uncomfortable from the medication.
Due to work commitments, I haven't returned for about six months now.
Currently, I still experience frequent urination, a feeling of residual urine after urination, and abdominal bloating.
Additionally, when I exert pressure on my abdomen, I feel a strong urge to urinate, and I constantly feel the need to have a bowel movement.
When I do have a bowel movement, a grayish-white fluid is expelled.
I am really troubled and unsure which organ might be the issue.
I am very concerned about the potential impact on my fertility.
Could you please provide me with some guidance on where to seek help? Thank you!

Dr. Liu Jiaju reply Urology
It is recommended to visit a urologist for a semen analysis.
This can help rule out any reproductive system infections and assess fertility status.

More Info
Frequent urination and associated symptoms such as urgency, discomfort, and the sensation of incomplete bladder emptying can be distressing and may stem from various underlying causes. In your case, considering the history of prostatitis and the ongoing symptoms, it is essential to explore several potential factors that could contribute to your condition.
Possible Causes of Frequent Urination in Men
1. Prostatitis: You mentioned a diagnosis of non-bacterial prostatitis. This condition can lead to inflammation of the prostate gland, causing symptoms such as pelvic pain, urinary urgency, and frequency. Chronic prostatitis can be particularly challenging to treat, and symptoms may persist despite normal test results.
2. Bladder Irritability: Conditions such as bladder overactivity or interstitial cystitis can cause frequent urination and a sense of urgency. These conditions may not always show abnormalities in standard urine tests but can significantly impact quality of life.
3. Neurological Factors: Issues with nerve signals that control bladder function can lead to symptoms of urgency and frequency. Conditions affecting the nervous system, such as multiple sclerosis or diabetes, can disrupt normal bladder control.
4. Psychological Factors: Anxiety and stress can exacerbate urinary symptoms. Psychological factors can lead to increased awareness of bodily sensations, including the need to urinate.
5. Dietary Influences: Certain foods and beverages, such as caffeine, alcohol, and spicy foods, can irritate the bladder and lead to increased urinary frequency. Keeping a food diary may help identify any dietary triggers.
6. Hormonal Changes: Hormonal imbalances, particularly involving testosterone, can affect prostate health and urinary function.
Recommended Diagnostic Steps
Given your ongoing symptoms, it would be prudent to return to a urologist for further evaluation. Here are some steps you might consider:
1. Comprehensive Urinary Assessment: A urodynamic study can help assess how well your bladder and urethra are functioning. This test measures the pressure in the bladder and the flow of urine.
2. Imaging Studies: An ultrasound of the bladder and prostate may provide additional insights into any structural abnormalities.
3. Further Blood Tests: Checking hormone levels, including testosterone, and assessing for diabetes or other metabolic conditions can be beneficial.
4. Psychological Evaluation: If anxiety or stress is suspected to contribute to your symptoms, speaking with a mental health professional may be helpful.
Treatment Options
1. Medications: Depending on the diagnosis, medications such as anticholinergics or beta-3 agonists may be prescribed to help manage bladder overactivity. If prostatitis is confirmed, antibiotics or anti-inflammatory medications may be considered.
2. Pelvic Floor Exercises: Engaging in pelvic floor exercises (Kegel exercises) can strengthen the muscles involved in urination and may help alleviate some symptoms.
3. Behavioral Therapy: Bladder training techniques can help retrain the bladder to hold urine longer and reduce urgency.
4. Dietary Modifications: Reducing intake of bladder irritants and maintaining adequate hydration can improve symptoms.
5. Physical Therapy: A specialized pelvic floor physical therapist can provide targeted exercises and techniques to relieve pelvic pain and improve bladder function.
Conclusion
Your symptoms are complex and may require a multifaceted approach to diagnosis and treatment. It is essential to communicate openly with your healthcare provider about your ongoing symptoms and concerns, including any fears regarding fertility. With appropriate evaluation and management, many individuals find relief from their symptoms and improve their quality of life. Don’t hesitate to seek a second opinion if you feel your concerns are not being adequately addressed.
